Job Summary:

As a Transplant Nephrology APRN/NP/PA, you will work in collaboration with the Transplant Nephrology physician faculty to provide outpatient clinical care to kidney transplant patients from the time of evaluation through long term post-transplant follow up. Joining the Transplant Nephrology Advanced Practice Provider (APP) team presents an exciting opportunity collaborate with highly skilled APPs, nephrologists, pharmacists and nurse coordinators to care for kidney transplant patients in a busy, high acuity transplant program.

Key Responsibilities:

  • Evaluate and manage patients following kidney transplant in the outpatient setting. This includes obtaining a thorough history, performing an appropriate physical examination, ordering appropriate diagnostic testing, providing documentation, as well as an assessment and care plan
  • Manage immunosuppression, infection and both medical and surgical complications post kidney transplant complications from immediate acute outpatient follow up through years post transplant
  • Manage hypertension, cardiovascular disease, diabetes, bone disease and chronic kidney disease in the post kidney transplant patient
  • Complete pre-transplant evaluations, waitlist evaluations for kidney transplant, kidney-pancreas transplant and living donation as a part of the multidisciplinary team
  • Independently manage patients at outreach clinics at UCH satellite locations
  • Participate in clinic sponsored educational opportunities.
  • Attend required meetings and participate in committees as requested.

Qualifications:

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.

-       An Advanced Practice Nurse or PA and nationally certified

-       Minimum 2 years experience as an APP is required  

Preferred Qualifications:

-       Transplant or nephrology experience preferred, but not required  

Knowledge, Skills and Abilities:

  • Excellent verbal and written communication skills to effectively interact with a diverse professional and patient population.
  • Knowledge of organizational policies, regulations and procedures to administer patient care. 
  • Knowledge of medical equipment and instruments to administer patient care. 
  • Knowledge of common safety hazards and precautions to establish a safe work environment. 
  • Skill in applying and modifying the principles, methods and techniques of medicine and surgery to provide on-going patient care. 
  • Skill in taking medical history, assessing medical condition and interpreting findings. 
  • Skill in preparing and maintaining records, writing reports, and responding to correspondence. 
  • Skill in developing and maintaining department quality assurance. 
  • Skill in establishing and maintaining effective working relationships with patients and their family members, medical staff, residents, fellows, medical students, and the public. 
  • Ability to maintain quality control standards. 
  • Ability to react calmly and effectively in emergency situations. 
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Ability to communicate clearly as an integral member of a team

    Anticipated Pay Range:

    The starting salary range (or hiring range ) for this position has been established as HIRING RANGE:

    $102,211 to $127,628 

    The above salary range (or hiring range ) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. Salary is commensurate with experience. This position may be eligible for overtime compensation, depending on the level.
