As a member of the multidisciplinary Hepatology team, this Social Worker will conduct comprehensive assessments for patients in the acute hospital or outpatient clinic setting diagnosed with ALD. Using a strengths-based model, this Social Worker will assess and identify formal and informal individual and community support options for sobriety and assist these patients to establish with substance use treatment in their communities. The Social Worker will assist the patient to outline interventions and ensure appropriate connection to resources for treatment of alcohol use disorder, as well as, obtain updated data from the treating community agency and communicate regularly with the Transplant and Hepatology multidisciplinary teams. The Social Worker will obtain collateral information family members, other involved support persons, treatment providers, lab results and medical records. This position requires up to date knowledge of area addiction medicine/chemical dependency resources and excellent clinical and organizational skills, with the ability to interact with patients, families, hospital and medical staff, students, and a wide range of external contacts.
