Position Description:

The Division of Infectious Diseases is seeking to hire a Medical Case Manager I. The Medical Case Manager I will report to the Senior Medical Case Manager and is vital to engage Ryan White Part B eligible newly HIV-diagnosed, Returning-to-Care (RTC), and transferring patients in the Kentuckiana Regional Planning and Development Agency (KIPDA) region, into appropriate primary health care. This position will work in methods in which an HIV diagnosis is delivered, and the way post-test counseling is provided to create a high linkage to care success rate.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Responsible for providing linkage to care support services to Justice Involved individuals identified in City/County facilities within the designated Service Area.
  • Provide Counseling and Testing support to help identify newly diagnosed individuals within the designated service area as part of the Linkage support team.
  • Provide effective Ryan White enrollment application linkage and retention care services to newly diagnosed, transferring intake staff, in order to maintain an efficient application process for all Part B eligible clients who are new, transferring or reengaging in Kentucky AIDS Drug Assistance Program (KADAP).
  • Support new/returning/transferring Intake patients in the provision of a range of client-centered support services such as medical care, psychosocial, housing, and other services.  
  • Coordinate with Ryan White Part C/D Linkage team and other Part B Medical Case Managers to ensure timely follow-up of inactive Part B client cases and collaborate with Ryan White Part C/D Case Management team to address new/returning/ transfer referral challenges as appropriate.
  • Maintain a caseload of 75+ clients by providing non-medical case management that includes treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
  • Conduct initial acuity assessment of client service needs; develop a comprehensive, service treatment plan and coordinate appropriate services to achieve objectives.
  • Re-evaluate and make adaptations to the service treatment plan, at least every six months, as necessary during the annual eligibility period.
  • Complete program eligibility documentation including HIV status/diagnosis, residency, and income every six months.
  • Ensure new patients are updated and documented in Careware case management data including types of services provided, encounters/ communication, duration, and frequency of the encounters.
  • Assists patients to access other public and private programs for which they may be eligible and coordinate services with community programs, vendors, and healthcare providers to effectively meet client needs.
  • Coordinate and follow up on medical visits, labs, and assessment of viral load suppression.
  • Provide clients with educational information regarding disease transmission and prevention methods and encourage a healthy lifestyle over the course of continued case management.
  • Participate in ongoing HIV education and attend Ryan White Part B services conference and seminars.
  • Participate in clinical quality management activities as assigned.
  • Other duties as assigned.
Target Compensation Maximum:

$31.38

Target Compensation Minimum:

$20.92

Compensation will be commensurate to candidate experience.
