Job Description Summary:
The Community Health Facilitator Manager will manage operations of the Facts Not Fear ICT project and will supervise three Community Health Navigators. This position supports the Project & Evaluation Lead. This position will work closely with community partners and other agencies to identify gaps in services to ensure cultural and linguistic appropriate services according to National Standards for Culturally and Linguistically Appropriate Services (CLAS) in Health and Health Care.
The Community Health Facilitator Manager will prepare financial and progress reports and maintain the budget, budgeting reports, and ad hoc financial reports as necessary.
This position will support the Facts Not Fear ICT project, which aims to advance health literacy, incorporate Wichita minority communities in all aspects of the work, empower health care providers to effectively communicate with patients, and increase vaccination rates for target communities. Facts Not Fear ICT ultimately aims to increase positive health outcomes for disparate communities in Wichita, Kansas. Specifically, Facts Not Fear ICT targets Hispanic/Latino, Black/African American, Asian, and American Indian and Alaska Native communities in Wichita. Lead Project Coordinator duties will include: team strategizing and planning to address social determinants of health barriers through advocacy, support, and education, management of community health navigators, and preparation of monthly and quarterly program reports as required by HHS-OMH grant initiative.
This position will require flexibility to work some nights and weekends, hours can be flexed within same week.
This position is full-time, 40 hours per week.
This position will pay up $63,000, commensurate with experience.

Required Qualifications:
  • Education:
  • Bachelor’s degree and/or a combination of education and relevant experience working on community-based projects
  • Work Experience:
  • Ability to work with email, internet, and Microsoft Office (Word, Excel, and PowerPoint)

  • 3+ years of experience working in budget management
  • Experience conducting community related work

Job Duties Outlined:

  • Provide oversight and supervision of Community Health Navigators
  • Work with Marketing Contractor to prepare marketing for Facts Not Fear ICT project
  • Manage operational activities of assigned projects
  • Oversee all contracts and financial reporting
  • Build and support clinic engagement
  • Work closely with clinic site leadership and project team to discuss any problems or concerns and problem solve collaboratively
  • Gather and organize information about the problems to be solved or the procedures to be improved
  • Assists with daily activities related to grant management to ensure compliance with state, local, federal regulations
  • Assist in coordinating the implementation of projects with the other partner agencies and organizations
  • Report project status to partners and funder
  • Monitor the progress of projects
  • Prepare project reports for Advisory Board presentations
  • Perform other duties of similar nature assigned by the grantee and/or subrecipient
