This role will have the opportunity to expand your current knowledge and gain new skillsets serving the community through integrated behavioral health. This role involves direct clinical intervention, acting as a liaison between patients, physicians, and community resources. It includes detailed case management evaluations and services across the lifespan. The professional assists with intake and referral systems in a clinic, contributing to training resident physicians in quality improvement efforts, particularly for Spanish-speaking populations. In terms of clinical service, the individual provides integrated behavioral health case management services, oversees training for medical and behavioral health providers in medical settings, and participates in community activities promoting healthcare. They share expert knowledge of community resources to facilitate transitions of care, provide family and patient education, and conduct workshops and seminars. The role also involves ensuring compliance by documenting services according to department standards, assisting with program evaluations to analyze service utilization and effectiveness, and performing assigned duties.

The Department of Primary Care and Rural Medicine is committed to full-scope family medicine and community care. Through our keen focus on rural populations and innovative care delivery, we’re eliminating health disparities throughout Texas and bringing health care where it’s needed most. We utilize our own clinical sites and a select group of clinical affiliates to provide excellent educational settings for our faculty, students, and residents for a community-based approach to preventive health care. 

Requirements

  • Master’s degree in social work

  • Two years of relevant social work experience

  • Texas Licensed Master Social Worker (LMSW), Licensed Professional Counselor Associate (LPC-A), or Licensed Marriage and Family Therapist (LMFT). Must maintain licensure for continued employment

Preferred qualifications include experience in integrated behavioral health and/or medical settings, case management in medical settings, brief behavioral interventions, telehealth, or related fields. Strong oral and written communication skills, literacy in technology usage, organizational flexibility , and the ability to speak English and Spanish are also desired.

The responsibilities include (but not limited to):

  • Clinical Intervention: Provides direct clinical services. Serves as a liaison between patients, physicians, and community resources. Provides detailed case management evaluations. Provides case management services throughout the lifespan. Provides assistance with intake and referral systems needs of the clinic. Assists with training resident physicians in quality improvement efforts to assist Spanish speaking populations with health literacy and healthcare needs.

  • Clinical Service: Provides integrated behavioral health case management services and oversees training for medical and behavioral health providers within medical settings. Participates in community activities which promote health care. Disseminates expert knowledge of community resources to coordinate transitions of care. Provides family and patient education. Conducts workshops and seminars as needed.

  • Compliance: Documents services and activities per department standards. Assists with program evaluation activities to track and analyze utilization and effectiveness of services. Performs other duties as assigned.
