UW Medical Center - Northwest has an outstanding opportunity for a Case Management Assistant in the inpatient setting. The Case Management Assistant will be responsible for working closely with the Social Worker, Nurse Care Coordinator, Physician, and the patient to arrange follow-up appointments post-discharge and assist with coordination of the discharge planning process for patients.

SCHEDULE:
Temporary/Per Diem, day shift, 8 hours per shift.
