Occupational Summary
Provide consultation and technica l support to families of patients receiving services within the Duke Uni versity Department of Psychiatry, specifically NC-PAL, regarding coordin ation of care across the School of Medicine (SOM) as well as available community resources.
Work Performed
Serve as a consultant and patient navigator to parents/caregivers of children and adolescents with IDD, Intellectual Development Disabilities via telephone and in clinic. Guide parents in navigating and coordinating clinical and educational services within the Duke University Systemand broader community. Assist parents with reaching their self-identified goals. Encourage parents to contact the appropriate medical and educational professionals when they have a concern. Schedule time and place for family activities and conduct follow-up phone calls to program families. Compile and keep records of information concerning services rendered to families .

Knowledge, Skills and Abilities
High level of knowledge regarding the resources available in the community for individuals with special health care needs and their families. Experience navigating youth-serving systems of care, including educational , health, supportive services, etc. Respectful of the differences in culture and language in families and in local communities. Ability to work with families, staff, and diverse communities. Ability to work with groups, as well as individuals, using a family-centered team approach.
Excellent communication skills, ethical, and bilingual-Spanish/Eng lish preferred.
Education
Work requires a general educational background normally equivalent to a high school education.
Experience
Experience with providing guidance and assistance to families of individuals with IDD.
