This is a patient facing position serving as a non-clinical resource and liaison for older adult patients and their caregivers. Assists with navigating all aspects of the healthcare system. Uses knowledge of the unit or clinic to resolve issues and identify ways to increase patient satisfaction. Collaborates with department managers to meet patient expectations, achieve quality outcomes and build a patient focused culture.

The ZSFG Age-Friendly Emergency Department (AFED) Initiative is a philanthropic foundation funded grant to implement age and dementia-friendly initiatives into the daily operations and practice of emergency medicine and nursing for patients cared for at ZSFG. Over the course of three years, the ZSFG Emergency Department will become a level one certified AFED offering age-friendly medical, nursing, and support services to older adult patients and those suffering with dementia. In addition, the AFED aims to link older adult patients and their caregivers with community resources that will result in improved outcomes, decreased mortality and morbidity, and reduced hospital admission rates. The ZSFG AFED initiative team consists of the Department of Emergency Medicine Medical Director, a AFED Medical Director, Geriatrician, Nurse Manager, AFED nurse practitioner, Project Manager, Data Analyst, and Patient Navigator. 

The Patient Navigator is responsible for the implementation project by providing support to nursing and social medicine resources, particularly with regard to linkage to designated treatment clinics, age-friendly community resources, and facilitating execution of treatment plans and filling medication prescriptions.  The Patient Navigator acts as a communication liaison between treatment providers and maintains ongoing relationships with/offers support to enrolled patients. The Patient Navigator will assist the analyst with tracking patient data, as well as working with the team’s Project Manager to complete documents needed for reporting to funders and accrediting bodies.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Department Description

The Department of Emergency Medicine (DEM) provides emergency medical care to over 150,000 patients annually at the UCSF Medical Center at Parnassus, UCSF Benioff Children’s Hospitals at Mission Bay and Oakland and Zuckerberg San Francisco General Hospital and Trauma Center. DEM administrative and research staff support the work of 85 full-time faculty and 70+ attending physicians, a residency program, fellowship programs and over 300 housestaff and students each year. Our vision is to be the leader in compassionate, innovative and equitable emergency care for all people at all times.

The population served by the DEM is medically, socially and ethnically diverse. ZSFG is the county's only trauma center, and UCSF provides the full resources of one of the nation's leading tertiary care referral centers.

The DEM and AFED Medical Directors are clinicians in the DEM and have experience in providing comprehensive care management to complex, underserved populations. The AFED initiative will also partner with a core team member in the Division of Geriatrics in the Department of Medicine, which provides Geriatrics expertise and consultation on linking patients and caregivers to an outpatient treatment program.


Required Qualifications

  • Bachelor's degree in related area and / or equivalent experience / training
  • Minimum 3 years of related experience
  • Solid interpersonal and customer service skills. Ability to be diplomatic and highly organized. Ability to remain calm under pressure and apply sound judgment
  • Ability to work independently in a fast-paced, demanding environment with minimal supervision. Ability to manage and oversee multiple tasks simultaneously, including high daily call volume
  • Ability to communicate effectively, both orally and in writing
  • Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area

Preferred Qualifications

  • Acquiring knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Knowledge of data collection, compilation, and analytical techniques
  • Skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. Working knowledge of the organization and how to get issues resolved
  • Ability to create effective rapport with a diverse group of patients and care providers
  • Proficiency in Spanish and/or Chinese dialect
  • Excellent analytical, interpersonal, and problem-solving skills
  • Experience working with electronic health records
  • Knowledge of local age-friendly resources
