Job Summary

Involves experience in providing patient care consultation and patient education related to wound and ostomy care. Ensures evidence-based practice for practice in the wound, ostomy, continence specialty area. Provides direct patient care, consulting, teaching, mentoring, and systems improvement leadership. Influences outcomes by providing expert consultation to all care providers and implementing improvements in health care delivery systems.

The Wound, Ostomy, Continence (WOCN) Nurse is a clinical leader that is responsible for prevention, planning and providing care for patients with complicated wounds, ostomies, fistulas, presssure sores and incontinence nursing care at UCSF Benioff Children's Hospital San Francisco (BCHSF). The advanced knowledge and skills required for this role include clinical expertise in a focus area, evidence-based practice, education, consultation, and leadership. The position works in collaboration with the Nursing Leadership, Faculty/providers, APNs and Nursing Staff at BCHSF. The WOCN will be responsible for the development of a comprehensive program for the prevention, care, education, and rehabilitation of children requiring wound, ostomy, and continence management.

The WOCN will perform initial and ongoing patient assessments and receive consult requests. The WOCN will collaborate to formulate individualized treatment plans based on the needs of the patient and family, and oversee implementation of the treatment plan. The WOCN will refine or develop as necessary patient education tools, materials for APEX that will facilitate the education of patients/families in the management of wounds, ostomies, tubes, drains and wound vacs. The WOCN will document face-to-face encounters with clinical notes in the medical record. Treatment plans will be coordinated with the UCSF discharge coordinators to better meet the needs of the patients, families, and referring providers after discharge. The WOCN may communicate with referring providers at the time of admission, discharge, and will coordinate community follow up to facilitate a successful transition.
The WOCN will work collaboratively with an interdisciplinary team that includes nurses, physicians, administrative staff, as well as allied health professionals to provide consistent care to each patient/family. The WOCN will assess the needs of the nursing and medical staff and provide educational events to meet these needs initially and ongoing.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Department Description

The Benioff Children’s Hospital San Francisco (BCH SF) is a regional tertiary center serving patient and families of all pediatric diagnosis and related conditions. Serving the 9 Bay counties and beyond, BCH SF is a regional leader in many clinical services. BCH SF is an Acute Care facility with 189 inpatient beds, a pediatric ED, a pediatric operating and recovery area. A Magnet Designation facility comprised of nursing services who provide comprehensive, compassionate and highly specialized care to the most fragile pediatric patients and their families. In addition to providing direct patient care, nurses participate on unit-level committees, ensuring that each staff member has an active voice in the operation and function of the unit.

Required Qualifications

• A minimum of 2 years of recent clinical RN experience in the specialty area
• Bachelor's degree from an accredited Nursing program
• Registered Nurse in the State of California
• Current American Heart Association CPR certification
• WOCN certification (may be in progress with completion within 1 year of hire)
• Advanced interpersonal skills to participate in the development and delivery of new education and training programs and to develop positive working relationships with nursing staff and leadership.
• Advanced skills in written and verbal communications to convey the most complex clinical, technical, and education and training information in a clear and concise manner, and to produce high-quality reports, documentation, and presentations.
• Advanced critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess highly-complex and urgent problems to develop an expedient path of resolution.
• Flexibility to orient and work at all UCSF Medical Center locations.

Preferred Qualifications

• Master's degree from an accredited Nursing program

License/Certification

• Registered Nurse in the State of California
• Current American Heart Association CPR certification
• WOCN certification (may be in progress with completion within 1 year of hire)
