Job Summary

The Transplant Infectious Disease (ID) APP, Physician Assistant (PA) or Nurse Practitioner (NP) hospitalist works collaboratively within a multidisciplinary health care team. He or she will be expected to perform various tasks on behalf of the hospital that are critically important for safe, efficient and patient centered medical care. The Transplant ID Hospitalist is responsible for coordinating the hospital’s effort to provide comprehensive care for hospitalized infected transplant patients including those who may be severely ill. He or she will also educate patients and their families in order to increase their satisfaction with their hospitalization and to promote wellness, prevent medical problems, facilitate patient services and intervene as needed.

Job Duties and Responsibilities

  • Work with the ID Fellow covering TxID1 (Transplant team #1) to triage consult requests between TxID1 and TxID2.
  • For all new consults, pre-round on the patient including examination and interview, gather and document background history, laboratory and radiographic data, including pertinent information from outside facilities (e.g. call outside facility to secure updated culture information, or transfer of radiographic imaging, etc.)
  • For all patients each day: 
    • Pre-round on each patient on the service, examining the patient and collecting and documenting the 24 hour clinical course, laboratory, radiographic, and culture data in order to facilitate sequence of rounds
    • Review and document all antibiotics including duration of therapy and preparation (e.g. oral, intravenous, inhaled, etc.), any dosing changes and drug side effects, interactions, & toxicity; emphasis will be placed on compliance with hospital infection care protocols and antibiotic stewardship including IV to oral switch, using Duke Hospital formulary medications, narrowing antibiotic focus, and discontinuing antibiotics as soon as clinically prudent
    • Screen each patient for ongoing antibiotic trials offered at Duke
    • Round daily with the TxID Attending
    • Present new consults to the TxID attending for review, final recommendations
    • Review old patients on the census including any new, pertinent updates
    • Communicate care plans to the primary transplant services.
    • Coordinate outpatient follow-up / transition to TxID clinic.
    • Arrange outpatient TxID clinic visits
    • Follow labs/drug levels, etc. in conjunction with inpatient attending until the patient is seen in TxID outpatient clinic

Job Eligibility Requirements

Education

PA:         Graduate of ARC accredited Physician Assistant program, Master’s Degree preffered

NP:         Graduate of MSN or DNP Nurse Practitioner Program for Adult Care

Certification

PA:         PA-C     

NP:         National certification to treat adults, e.g.  FNP, ANP, ACNP, AGNP, AGACNP

Licensure 

PA:         Current inhibited medical license issued by NCMB with authorization to practice as a PA

NP:         Current uninhibited RN license issued by the NCBON with authorization to practice as an NP

Experience

Prefer 1-2 years of experience in a similar setting, but not required.         

Physical and mental requirements:

  • Able to clearly articulate scope of practice, and practices within those guidelines as a Physician Assistant in the state of North Carolina and within the Duke University Health System.
  • Able to independently seek out resources and work collaboratively to solve complex problems
  • Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others.
  • Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up.
  • Able to use fine motor skills
  • Able to record activities, document assessments & interventions; prepare reports and presentations
  • Able to use computer and learn new software programs
  • Able to provide leadership in clinical area of expertise and in meeting organizational goals
  • Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations
  • Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds
  • Able to remain focused and organized
  • Able to work collaboratively with all levels of personnel

Job Hours

Primary hours will be four 10-hour shifts per week, and other hours as needed to fulfill the responsibilities of the position.  Primary hours will be flexible to cover the needs of the service.
