Transplant Coordinator – Duke University Hospital – Day Shift

 Duke University Health System- Duke University Hospital  seeks to hire an experienced Transplant Coordinator who will embrace our mission of Advancing Health Together.

 Department Profile:

The Kidney Transplant Department at Duke Health is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time.   

• Educate patients and families regarding all aspects of transplantation care, including but not limited to, arranging follow-up appointments, labs, and working with the multidisciplinary team on patient-specific needs in the post-transplant phase.
• Communicate with patients and caregivers regarding the plan of care and ongoing changes.
• Designated caseload of post-transplant patients to provide a continuum of care. 
• Provide for continuity of care between dialysis, outpatient care, and long-term management. 
• Ability to make clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
• Collaborate with medical faculty, pharmacy, nursing personnel, case management, and other health disciplines to facilitate meeting patient and family needs.
• Staying current on transplant research and applying evidence-based research and recommendations to patients/families on the care of the transplant patient.
• Complete regulatory documentation in UNOS database/hospital EMR.
• Reviewing education materials and working with the department if there are changes in current practice.
• Management of immunosuppression plan per protocol/ medical team.  Ongoing education with patients and caregivers.

 This position does require on-call responsibilities shared with a large team for organ offers for adult / pediatric kidney and kidney-pancreas candidates. 

This is a salaried position in a team of 5 Kidney coordinators and 1 inpatient coordinator dedicated to post-transplant care. 

The transplant coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, and teaching and in providing leadership in the organization.

Education
BSN required MSN preferred.

Experience
5 years nursing experience with at least two years in transplant, critical care, disease-specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease-specific.

Degrees, Licensures, Certifications
Current or compact RN licensure in the state of North Carolina required. BLS required.
