Care Transition Management will lead teams across UCD Health Systems by developing programs for clinical services, education and training with local and regional post-acute care partners including skilled nursing facilities, long-term care hospitals, rehabilitation hospitals, psychiatric hospitals, home health agencies, hospice programs, and federally qualified healthcare clinics. The goal is to enhance the experience and clinical outcomes of patients by ensuring seamless patient flow as they are discharged from the hospital.

The Transition of Care (TOC) Coordinator (Pat Navigator Non-Clinical 4) will provide support services in all functional areas of the TOC objectives and goals in an inpatient, outpatient and community settings. The TOC Coordinator will serve as the communication link between patients and healthcare professionals and act as an expert resource person for all transitional care needs. Using a high degree of knowledge and expertise in problem solving and analysis of unique challenges related to transition of care of patients across the health care continuum, this position works collaboratively with UC Davis Health Plans, other Health Plans, health care professionals and agencies to ensure a smooth transition and improve delivery of services to the community including coordination of all TOC care coordination management and activities.
Required Qualifications
  • Advanced interpersonal, written and verbal communication and interpersonal skills with ability to resolve issues effectively with a diverse population of patients, staff and physicians.
  • Excellent customer service skills to understand and articulate diverse clients' needs and objectives; manage expectations and deliver services in a timely manner.
  • Thorough knowledge of community resources, insurance authorization process, discharge planning and best practices.
  • Strong critical thinking and problem-solving skills to manage complex information, assess problems, and develop effective solutions.
  • Must be able to communicate effectively with diverse audiences including physicians, nurses, allied health professionals, administrator and a wide-variety of non-technical staff.
  • Excellent planning, organizational, and time-management skills.
  • Skills, knowledge and ability to support the objectives of TOC and the policies and procedures of UC Davis Health System.

Preferred Qualifications
  • Minimum two years (2) experience working in a health care organization.
  • Minimum two years (2) experience in acute or post-acute discharge planning or case management. 
  • Experience in a large academic healthcare setting.
  • Bachelor's degree in healthcare management, public health, social work or other healthcare or related.

Special Requirements
