Job Description:

Activities can include but not limited to any or all of the following depending on the need of the Liberty Partnership Program:

  • Become familiar with the nature of the LPP
  • Become familiar with LPP policies and procedures.
  • Work collaboratively with participating LEAs as assigned.
  • Engage the identified student and their parent/caregiver to enroll them in the Liberty Partnership
  • Program
  • Perform outreach and engagement for all LPP referrals assigned, including but not limited to:
  • Conduct social emotional assessment approved by NYSED and incorporate findings into Personalized Learning Plan and other goal-minded activities.
  • Completing applicable referrals based on need (counseling, primary care, social determinants
  • of health etc.)
  • Providing outreach and engagement to enroll referred individuals into the Health Home program.
  • Collaborating with providers, family, and school faculty to establish appropriate support networks as needed.
  • Follow-up communication with school referent regarding outcome of referral and with student and family to ensure successful service connection.
  • Works collaboratively with the student college interns (mentors and tutors) to deliver programming that offers career and college readiness opportunities.
  • Works collaboratively with the SUNY Polytechnic Career Services to enroll parents and students in
  • the college sponsored workshops
  • Works collaboratively with the SUNY Polytechnic Summer Program to deliver proposed
  • Programming
  • Works with school district staff, including but not limited to district and building administration,
  • guidance, and social work staff, to learn district wide policies and procedures, as well as building specific policies and procedures.
  • Attends school meetings as needed.
  • Become familiar with existing school resources and programs.
  • Attends and completes all training and certifications as required by the LPP.
  • Attend LPP Advisory Board Meetings and all other appropriate community-based meetings as assigned.
  • Collect, track, and maintain LPP data with the utilization of project-specific trackers, SchoolTool,
  • and other applicable resources and produce appropriate weekly and monthly outcome data as directed
  • by the Project Director of LPP.
  • Traveling between district sites, administrative office, and/or community locations as designated by
  • the Management Team for in person meetings and/or trainings.
  • Participation in District/Community Wide Programming events
  • Additional duties assigned as applicable to the needs of the district and/or LPP.


Requirements:

  • Bachelors of Arts or Science Degree; preferably in education, psychology, sociology, human services or related field with two years of relevant experience.
  • Basic Computer Skills (Windows, Outlook, Word, Excel)
  • Travel is required. Must have a valid NYS Driver's License.

  • Complies with quality assurance, OSHA, HIPAA, FERPA (Family Educational Rights and Privacy Act), infection control, safety and other policies set forth.
