Social Worker (Psychiatry - TCC/NOW)

Job Summary:

Responsible for performing extended social work program development, case management, clinical research, outreach, patient assessment and evaluation requiring advanced skills and training.

 

1.     Job Duties:

2.     Performs psycho-social assessments, health education, supportive counseling and referrals; provides education on social services and public systems; assists patients in finding resources.

3.     Interprets and assists in resolving social problems related to medical conditions and/or hospitalizations.

4.     Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.

5.     Coordinates clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs as identified; formulates candid and supportive communication.

6.     Coordinates ongoing counseling and support groups; facilitates effective problem solving and conflict resolution.

7.     Assists with program evaluation activities including but not limited to data collection for grants, grant reporting, patient satisfaction surveys, and other activities as needed.

8.     Performs all other duties as assigned.

 

Education:

Bachelors degree in Social Work is required.

Experience:

One (1) to three (3) years of professional social work experience is preferred.

Additional Licenses and Certifications:

Licensed as a Social Worker in Texas (LMSW, LSW).
