Social Worker MSW (Psychiatry- TCC/NOW)

Job Summary:

Provides professional skills and advanced social services. Responsibilities include performing extended social work program development, case management, clinical research, outreach, patient assessment and evaluation requiring advanced skills and training.

 

1.     Job Duties:

2.     Performs psychosocial assessments, health education, supportive counseling and referrals; provides education on social services and public systems; assists patients in finding resources.

3.     Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.

4.     Identifies obstacles to optimal patient care; loss or grief work; risk reduction; and health promotion.

5.     Provides clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs as identified.

6.     Formulates candid and supportive communication.

7.     Coordinates ongoing counseling and support groups.

8.     Facilitates effective problem solving and conflict resolution.

9.     Participates in program evaluation activities including but not limited to data collection for grants, grant reporting, patient satisfaction surveys, and other activities as needed.

10.  Performs all other duties as assigned.

 

 

Education:

Master's degree in Social Work is required.

Experience:

Three (3) years of professional social work experience is required. Experience with community resources and services for those with serious mental illness preferred.

Additional Licenses and Certifications:

License as a Social Worker in Texas (LMSW, LSW) is required.
