Job Summary 

Offer social work services/support and care coordination to cystic fibrosis patients and families from birth through adulthood.

Essential Functions:

CLINICAL RESPONSIBILITIES/ CARE COORDINATION:

  • Function under the direction of the CF Center Director
  • Offer social work services/ support and care coordination to cystic fibrosis patients and families from birth through adulthood.
  • Conduct ongoing psychosocial evaluations both inpatient and outpatient.
  • Conduct assessments and appropriate screening, including Mental Health screens, as per Cystic Fibrosis Foundation (CFF) Mental Health Guidelines) and PA Department of Health (DOH) quality of life screens.
  • Provide assessment through interview and observation to determine the need for social work services in accordance with departmental policy and standards of professional review.
  • Record all pertinent psychosocial notes in the patient records in a timely fashion. For outpatient records to finish within 1 week; for inpatient records to finish documentation by 24 hours.
  • Work within the team with newly diagnosed patients and their families to offer support around coping with the diagnosis of CF as well as other supports including but not limited to financial issues.
  • Establish adequate treatment plans according to standard professional review.
  • To consult and provide ongoing support during hospitalizations.
  • Provides counseling and assistance to patients and families during admission, hospitalization, and discharge.
  • Work with the Inpatient Pulmonary team to coordinate inpatient care.
  • Coordinate parent and medical staff meetings.
  • To be available for scheduled CF outpatient office visits.
  • Prioritize, coordinate, evaluate and recognize learning needs of patients and families.
  • Complete all necessary documents and stats for screens.
  • Assess Social Determinants of Health (SDoH)
  • Complete document in EPIC
  • Identify and coordinate with the CF team to eliminate barriers to care.
  • Complete all paperwork required for PA DOH Client Assistance Fund, if applicable
  • Provide brief psychosocial counseling to patients and families, as dictated by the Cystic Fibrosis Foundation Care Center guidelines.
  • In consultation with the CF team to counsel patient re: college, job skills.
  • Liaison to community agencies and referral sources for patients and families requiring those services.
  • Responsible for transition guideline to an Adult CF Center or other CF Center
  • Responsible for "no-shows" and cancellations.
  • Assist in ensuring appointments are within the CFF guidelines or needs of the patient.
  • Identify barriers to care (see 2.xii.3)
  • Write letters for CF patients, such as in diagnosis or other letters for insurance, letters related to school, such as 504 plans, IEP, travel, and any other services that the patient may need. Write letters for caregivers, such as work-related letters or completion of FMLA paperwork.
  • CFF Experience of Care (XoC): Lead ("Champion" as per CFF) on project for surveys regarding care within our Center.
  • To attend all XoC meetings (virtually)
  • Provide data to CF Center Director quarterly.
  • Other duties as per the Social Work Department and the Section of Pulmonology.

ADMINISTRATIVE:

  • Maintain and update SCHC CF Resource Binder annually and as needed.
  • Liaison to and collaborate with the local Delaware Valley Chapter Cystic Fibrosis Chapter and the National CF Foundation around patient education/ marketing initiatives, along with other members of the CF team as appropriate.
  • Be a functioning multidisciplinary team member, regularly attending and contributing to team conferences.
  • Attend all CF Team meetings as per meeting schedule.
  • Organize and facilitate CF Parent/Patient Advisory Committee and Meetings as per CFF and PA Department of Health requirements.
  • Participate in all Quality Improvement (QI) Initiatives including being the lead in a QI initiative.
  • Responsible for data entry to maintain PA Department of Health (State) grant status along with the CF Center Director.
  • Work with the Nurse Coordinator and other CF team members, when appropriate, to develop educational material to enhance family/ patient coping and compliance.
  • Participate in ongoing continuing education.
  • Attend the Annual North American Cystic Fibrosis Conference
  • Attend the Annual Regional MidAtlantic Conference
  • Attend the Annual CFF Social Work and Dietitian Consortium Conference
  • Prepare and distribute quarterly newsletters, with input from _CF team members.
  • Plan and execute Annual Family Education Day, if applicable

Required Qualifications 

  • Minimum of a Masters Degree or Equivalent . 
  • Minimum of 2 years’ experience.
  • Demonstrated effective communication skills in a multi-disciplinary setting.
  • Computer skills- experienced in Microsoft word, excel, email/outlook.

Physical Requirements 

  • Typically sitting at a desk/table.
  • Typically standing, walking.

Additional Information 

This position is classified as Exempt, grade K. Compensation for this grade ranges from $ 54,630 to $ 81,940 per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
