Position Summary
THIS IS A TIME-LIMITED POSITION WITH AN ESTIMATED DURATION OF 2 YEARS.
 
UNC Horizons is a trauma-informed hub-and-spoke model substance abuse treatment program. The home/hub for this permanent position will be the UNC Horizons Program at Wake. UNC Horizons Program at Wake provides comprehensive substance abuse treatment for women-identified people and their children residing in the Wake Co. area, including individuals transitioning out of correctional settings. This role is up to 50% of telebehavioral remote work and 50% on-site care coordination including pre-release sessions to initiate services and in-person post-release support. The purpose of this position is to provide care coordination as a Clinical Case Manager and for people transitioning out of North Carolina Correctional Institute for Women in Raleigh (in-person services). This position will work closely with case managers, social workers, and other behavioral health providers at NCCIW. The remaining 50% of this position will support reentry efforts for individuals leaving the North Carolina Correctional Institute for Women in Raleigh. This position will assist pregnant, postpartum, and parenting people to access transportation, housing, medication to treat opioid and alcohol use disorder, prenatal and primary care, and access to maternal-child outpatient and residential treatment and other interventions in North Carolina. Specifically, the purpose of this role is to build relationships with community stakeholders and communities and engage in community outreach. An ideal candidate will reside in the Wake Co area and would be available to facilitate trauma-informed telebehavioral health in a correctional setting.
Provide intensive case management services to clients leaving the North Carolina Correctional Institute for Women.
a) Engage in ethical service delivery and practices as required by all auditing bodies (certification and licensure boards, UNC University, UNC Horizons)
b) Coordinate with Correctional programming staff to initiate referrals to Horizons’ post-release SUD services and serve as a member of their interdisciplinary team
c) Meet with clients to orient them to services to identify needs and treatment goals pre-lease to promote continuity of care.
d) Meet clients at the time of release in a state vehicle to support them 
e) with transportation and continuity of care to access MOUD and SUD treatment, and other immediate post-release needs. 
f) Provide referrals to address physical and behavioral health needs, and social drivers to meet treatment goals, including assistance with obtaining health insurance, TANF/Workfirst, and other DSS-related resources
g) Provide advocacy to promote maternal-child reunification or visitation, when appropriate
h) Provide evidence-based behavioral health and counseling interventions 
i) Attend Horizons’ weekly interdisciplinary meetings and individual supervision with Program Director, and other team meetings.
j) Document services delivered in adherence to program productivity standards. All contacts for case management must be documented with a daily service note. The note must indicate: purpose of the intervention, the specific intervention providing how effective the intervention was, and how much time was spent providing the intervention.
Minimum Education and Experience Requirements
Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.
Required Qualifications, Competencies, and Experience
Excellent verbal and written skills.
Preferred Qualifications, Competencies, and Experience
Knowledge of the population served 
Preferred – History, knowledge, and of exposure to social work, psychology, sociology, criminal justice, human services field, or counseling experience; or experience with the substance abuse population, gender-specific treatment, child and family counseling, justice-involved population and a perinatal setting preferred.
Preferred: Qualified Substance Abuse Professional or CSAC (Certified Substance Abuse Counselor) or CADC (Certified Alcohol and Drug Counselor) or another relevant credential. An ideal candidate would express keen interest in criminal justice, substance use, and maternal health and be familiar with the resources in the in or near Wake County. Additionally, an ideal candidate would possess qualities such as working independently, being organized and have time management skills, communicating effectively, and having the ability to build rapport and new relationships while working in a hybrid setting.
In addition, the following base is expected:
1. Knowledge of counseling theory and practice.
2. Skill in assessment, case management, advocacy, and individual and group therapy.
3. Knowledge of the needs and concerns of women, including some exposure to issues such as childhood and adult trauma, domestic violence, sexual assault, substance abuse., pregnancy/parenting, and legal and economic difficulties.
4. Ability to work with women from varying incomes, ages, ethnic/racial groups, and lifestyles.
5. Knowledge of residential environments
6. Knowledge of supervisory role, especially with paraprofessionals.
7. Specialized knowledge of motivational interviewing, 12-step Philosophy, trauma therapies, and recovery principals
