Job Summary

The incumbent will be assigned to the Citywide Case Management CASC Program in partnership with the San Francisco Adult Probation Department (SFAPD) to provide services at the Community Assessment Service Center (CASC).   The incumbent will assist in providing comprehensive assessments, crisis intervention, case management, individual and group therapy, substance abuse services and making referrals as needed to a caseload of 25 clients. The incumbent will assist the CSWs in advocacy with the collaborative courts with dually diagnosed individuals. The incumbent will assist the program with CASC activities and outings. Clients will receive their mental health and substance abuse services at 564 6th street.  This program is part of the ZSFG Department of Psychiatry’s comprehensive effort to treat high risk clients in the community and to reduce unnecessary use of institutional care.

Department Description

The UCSF Department of Psychiatry at Zuckerberg San Francisco General Hospital and Trauma Center is dedicated to providing culturally competent services to the diverse patient population we serve. Providing a range of clinical services informed by evidence-based practices, the Department of Psychiatry provides care to thousands of patients each year. The department is divided into seven subdivisions. Our programs are staffed by UCSF faculty physicians and psychologists who lead multi-disciplinary teams of nurses, pharmacists, social workers, and occupational therapists in providing high quality care of patients and families experiencing acute mental illness. 

Since 1981, the Division of Citywide Case Management Programs has operated under the direction of the Department of Psychiatry at Zuckerberg San Francisco General Hospital and Trauma Center. Its mission is to support the recovery of San Francisco’s highest risk mentally ill adults and to reduce their use of institutional and acute care (psychiatric emergency services, hospital care, and/or jails), while helping to maximize their ability to maintain stable, productive, fulfilling lives in the community. All division program services include medication assessment and management, crisis intervention, outreach and case management, and individual, group and family therapy, which continue as long as clients need an intensive level of intervention. 


Required Qualifications

  • BA/BS degree in Social Work or related field and one year of related experience or equivalent combination of education and experience in the field of mental illness or criminal justice case management.
  • Case Management experience
  • Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology
  • Excellent communication and interpersonal skills
  • Able to work as a team member in a culturally diverse setting
  • Able to lift 50lbs and climb stairs
  • Demonstrated excellent attendance and reliability
  • A preliminary job offer and assignment may be extended pending successful background clearance.
  • An understanding of mental health issues and sensitivity toward mental health clients
  • Ability to work with clients and program staff from diverse cultural and social class backgrounds
  • Experience working with severely mentally ill adults, the homeless and dual diagnosed populations
  • Experience doing community based case management and the ability to do outreach including to SRO hotels
  • Experience working with individuals actively involved in the criminal justice system
  • Ability to obtain San Francisco County Jail Clearance through the San Francisco Sheriff’s Department
