Job Description Summary: The Nurse Home Visitor (NHV), under general supervision, provides comprehensive nursing services to women and families participating in the Nurse Family Partnership program (NFP).

This position is funded from grant and/or contract funding which is renewed under the provisions of the grantor of the contract. This position, therefore, is contingent upon continuation of funding of these grants and/or contracts and satisfactory job performance.

Required Education:

  • Bachelor’s Degree in Nursing

Required Experience:

  • Minimum of two years of clinical nursing experience

Required Licenses and Certifications:

  • Requires an active license in good standing as a Registered Nurse in the state of Texas; current CPR certification; valid Driver’s License

Required Special Knowledge, Skills, and Abilities:

  • Ability to multi-task and work cooperatively with others
  • Ability to communicate effectively, both written and verbal
  • Ability to work a flexible schedule

Preferred Education:

  • Master’s degree in Nursing or related field

Preferred Experience:

  • Experience in maternal/child health, public/community health, mental health, home visits
  • Experience working with diverse and underserved populations, communities, and organizations

Preferred Special Knowledge, Skills, and Abilities:

  • Bilingual in English/Spanish preferred

Job Responsibilities

Comprehensive Nursing Services

  • Maintains highest standards of nursing practice
  • Participates in Reflective Practice
  • Provides comprehensive nursing services to women and families participating in the Nurse Family Partnership program (NFP) in accordance with Professional Nursing Standards while also adhering to NFP model of practice
  • Drive to homes of families participating in NFP in order to provide vital services
  • Carries caseload of up to 30 clients

Reporting and monitoring 

  • Evaluates and reports client progress
  • Visits, monitors, tracks, and reports client data

Other General Responsibilities

  • Serves as a key member of the NFP program team
  • Participates in community coalition
  • Engages in NFP program team meetings
  • Promotes the Texas A&M NFP program via marketing and outreach efforts at local and regional levels
  • Completes required trainings and participates in professional development

  • Performs other duties as assigned.
