Posting Summary
Rutgers, The State University of New Jersey, is seeking a Resource Coordinator for NJMS- Infectious Diseases Practice. The position is responsible for conducting chart audits, quality improvement and quality assurance activities, assisting with training of staff, and helping to identify ways to improve programmatic efficiencies and outcomes. The resource coordinator also provides client-level support to clients living with HIV/AIDS with medical case management (MCM) services as defined by the Health Resource Services Administration (HRSA). Key MCM activities include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) provision of services required to implement the plan; (4) monitoring to assess the efficacy of the plan; and (5) periodic re-evaluation and adaptation of the plan as necessary over the life of the client.
Among the key duties of this position are the following:
  • Provides day to day guidance to the program’s medical case managers as it relates to client access to community resource
  • Performs a biopsychosocial assessment and develops a care plan to address the client’s needs initially and at a minimum every six months.
  • Answers client telephone calls. Correctly processes information for appointment scheduling, answer questions, and assist with problem solving. Screens emergent/urgent phone calls, inquiries related to the availability of appointments, scheduled appointment dates and times. If necessary, transfers calls to an appropriate person or department to assist the client.
  • Ensures referrals are scheduled in a timely manner documenting results to MCM and physician.
  • Communicates with physicians in a professional manner
  • Documents all encounters accurately in the electronic medical record (EMR) within 3 days of service.

Minimum Education and Experience
  • Bachelor’s Degree in a Health, Public Health, Social Work, or other relevant field required, plus three (3) years of relevant experience.
  • Equivalent education, experience, and/or training may be substituted for the degree requirements
