Position Summary:
Independently provides professional social work services to assist patients and their families with connecting to long term care and community resources while patient is receiving mental health services in the TCHATT
This position is for Texas Child Health Access Through Telemedicine. It’s currently a remote role but employee may be required to come into the office periodically for trainings, meetings, etc. Must be located in the Greater Houston/commutable area in order to work with local patient population and schools.
Preferred Skills: 1-3 years of experience as a Bilingual LMSW Social Worker experienced working with children and adolescents
Position Key Accountabilities:
1. Performs patient/family assessment of aftercare needs and develops a plan of care with multidisciplinary team in a manner that promotes quality patient care and customer service/satisfaction.
2. Manages patient's transition through the TCHATT system and exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated.
3. Obtains data from patients and/or relatives in the best interest of their problems to assist in the development of a number of significant teaching endeavors, programs, and social services.
4. Educates the staff on case management and provides specific information related to case types.
5. Evaluates patients’ responses and outcomes to education and therapeutic interventions
6. Maintains all documentation in multiple data management systems in a timely manner
7. Provides education and presentations to community providers and other healthcare providers on community resources and the impact of psychosocial factors on care need.
8. Utilizes crisis intervention, conflict resolution, and mediation to address patient/family concerns.
9. Responds to emergency situations.
10. Performs other duties as assigned.
Certification/Skills:
Licensure as a Bachelor Social Worker (LBSW) by the Texas Board of Social Work is required.
Licensure as a Master Social Worker (LMSW) by the Texas Board of Social Work is preferred.
Minimum Education:
Bachelor Degree in Social Work or related field.
Minimum Experience:
One year of social work experience required.
Medical social work experience is preferred.
Physical Requirements:
Exerts up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
