Job Summary

Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Rheumatology Nurse Care Coordinator (CNII) works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting.  The Ambulatory Rheumatology Nurse Care Coordinator (CNII)  will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Ambulatory Rheumatology Nurse Care Coordinator (CNII) will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse II will assist in the development of patient care standards and practices.

The Ambulatory Rheumatology Nurse Care Coordinator (CNII) is responsible for coordinating outpatient services and needs for a panel of pediatric Rheumatology patients and providers in the ambulatory setting. Their responsibilities include but are not limited to assisting with insurance authorizations and medication prior authorizations, managing and coordinating CCS comprehensive panel visits, providing comprehensive patient/parent education/teaching, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, and assisting in the overall coordination of timely patient care.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Required Qualifications

  • Minimum of one year full-time RN experience in pediatric inpatient setting
  • Current licensure as a Registered Nurse as defined by the California Board of Registered Nursing.
  • Bachelor’s of Science degree in Nursing (BSN)
  • Current CPR/BLS certification
  • Knowledge and ability essential to the successful performance of the duties assigned to the position.
  • Recent experience in the use of electronic medical record documentation
  • Recent experience with managing patients in the outpatient setting
  • Experience in managing Central line access and care, drawing labs
  • This position requires flexibility to orient and work at all UCSF Medical Center locations

Preferred Qualifications

  • Experience and fluency in working with computers
  • Knowledge of UCSF and APEX

License/Certification

  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR Certification
