Summary

The Texas Nurse-Family Partnership pairs Bachelor-degreed registered nurses with low-income, first-time mothers to improve prenatal care and provide one-on-one child development education and counseling. Women are enrolled early in the second trimester of their pregnancy, and a nurse begins visiting them. Visits, which usually occur in the family's home, continue throughout the teen's pregnancy and until her child is two years old. The nurses provide support, education, and counseling on health, parenting, developmental issues and life skills.

 

The ideal candidate for will be able to carry a caseload up to 30 clients, will work assignments will involve travel throughout designated areas to multiple client homes within Harris County, will have a reliable and insured automobile, has effective written and verbal communication skills are essential for interactions with families, colleagues, and stakeholders, has basic computer skills and is able to travel to Denver, Colorado for all face-to-face education sessions.


Job Duties
  • Assesses, develops, implements and monitors a comprehensive plan of care including goal setting at each visit and completes all required health, demographic, nursing assessments, mental health, developmental, and mother-baby dyad behavioral assessments, activities, teaching, recommendations, and nurses notes in agency, state, and national data collection systems.
  • Comprehensively assesses client's (as well as the child's) biophysical, psychosocial, and environmental needs.
  • Collaboratively identifies problems/barriers/opportunities for intervention. Facilitates goal setting, resolution and revision of plans on an ongoing basis.
  • Routinely assesses client's status and progress; if progress is static or regressive, determine the reason and proactively encourage appropriate adjustments in the care plan, providers and/or services to promote better outcomes.
  • Communicates thoughtfully and effectively with clients related to appointments, home or alternate visits, cancellations, re-schedules using approved methods considering HIPAA considerations at all times.
  • Participates in NFP team meetings, case conferences, program evaluations and data reviews to provide feedback, assess effectiveness, generate ideas, and develop plans to increase efficiencies and improve quality for both the NFP program and to clients/individuals enrolled in the program.
  • Stays informed of current trends in Maternal/Child nursing to provide safe, quality nurse home visiting services includes maintaining required certifications, licenses, or skills check offs and actively participates in shared learning during team based meetings.
  • Partners to provide community education, marketing and outreach for Nurse Family Partnership when needed for community awareness about the NFP program and to help generate referrals for sustainability and caseload management.

Minimum Qualifications
  • Associate's Degree or Bachelor's Degree in Nursing.
  • Two years of relevant experience as a licensed Registered Nurse (RN). 
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
  • Basic Life Support (BLS) Certification.

Preferred Qualifications
  • Experience working with teens and young adults.
  • Home visiting experience.
  • Bilingual in English/Spanish.
