Job Summary 

Responsible for the overall clinical success of the Ambulatory Internal Medicine Practice to ensure safety and compliance.

Essential Functions 

  • Support and participates in the Patient-centered Medical Home (PCMH) concepts of team- based care.
  • Staying up to date with PCMH guidelines
  • Assess, plan, implement, coordinate, monitor and evaluate healthcare options and services with the goal of increasing the likelihood of improvement to the health status of patients.
  • Provide outreach for hospital discharge follow-up (TCM), care planning, and coordination of services.
  • Educates Medical Assistants about PCMH requirements and how to meet them.
  • Collect data on quality metrics and track as required for improvement efforts.
  • Collaborate with patient’s medical/health providers to establish mutual goal setting including patients and their families/caregivers, utilizing self-management tools.
  • Does Blood Pressure Checks for uncontrolled hypertensive patients in the practice.
  • Educates uncontrolled diabetic patients about how to manage their sugars and achieve good glycemic control with A1c below 8.
  • Monitor patient’s INRs and promptly inform PCP when the INR is out of range.
  • Assists patients with access to medical supplies including glucometer, nebulizer, cane, walkers, wheelchairs.
  • Assist with coordinating referrals to homecare services.
  • Assist with unstable patients waiting for 911 to arrive.
  • Develops proper clinical procedures that ensure patient safety, comfort, and quality.
  • Evaluates ongoing patient care through incident reports and enacts appropriate actions when necessary and in accordance with all office policies and procedures.
  • Serves as resource and role model for staff.
  • Recommends and implements new initiatives when appropriate.
  • Reviews insurance registries including IBC/Aetna/Cigna to ensure missing gaps are addressed.
  • Assures compliance with all Clinic policies and procedures and governmental regulations pertaining to, but not limited to, the Pennsylvania Department of Health, AAAHC
  • Maintains professional licensure.
  • Stays up to date on all DOH, Medicare, AAAHC and other voluntary and regulatory requirements.
  • Establish and maintain effective working relationships with employees, managers, and physicians.
  • Assists with the development of patient education material.
  • Supports and promotes a positive image of the practice when dealing with patients and others inside and outside the organization.
  • Perform other duties as assigned.

Required Qualifications 

  • Minimum of a Bachelors degree in Nursing or Equivalent   
  • Minimum of 3 years of experience.

Certification/Licensure:

  • Current Pennsylvania State Nursing License.
  • CPR certification for Health Care Providers.

Preferred Qualifications 

  • Excellent organizational and interpersonal communication skills.
  • Demonstrated ability to manage multiple priorities.
  • Ability to write reports, business correspondence, and letters to patients.
  • Ability to work effectively and respond to questions from groups of patients and physicians..

Physical Requirements 

  • Typically sitting at a desk/table.
  • Typically standing, walking.

Location

Additional Information 

This position is classified as Exempt grade K. Compensation for this grade ranges from $54,630 to $ $ 81,940.00 per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
