As a Per Diem Registered Nurse, Care Coordinator at Cedars-Sinai you will be scheduled to work a minimum of 4 shifts per month on weekends. Responsibilities include the following.

  • You will serve as a clinical co-pilot—charting the best trajectory of each patient’s care by accommodating various needs, including medical, psycho-social, financial and practical.

  • You will coordinate the care and service of the patient population from admission through discharge.
  • You will assess and Plan, implement, monitor, and evaluate options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate level of care.

Educational Requirements:

BSN preferred. Associates Degree in Nursing required, CA License required

Licenses:

Experience and certification in Case Management preferred
Case Management certification expected within 2 years of hire
BLS Certified

Experience:

3 years minimum in Acute Care Nursing

Physical Demands:

Standing, Walking
