The Health Science Center (HSC) is looking for a program coordinator to serve as a member of the Primary Care & Population Health team. Job Description Summary: The Program Coordinator II (PCII) is responsible for coordinating all logistics, staffing, and patient scheduling for an externally funded cancer screening and prevention program (Texas C-STEP) offering free mobile mammography, well-women’s clinics at partnering facilities in service area counties, and cancer screening services provided through Texas A&M Health Family Care Clinic. This involves site visits; coordination with host site, technician vendors, and Texas A&M; and accurate post-event data management. The PCII works closely with the Program Manager, assisting in training and supervision of undergraduate student workers and community health workers (CHWs), and coordination of CHW activity on grants. The PCII works closely with healthcare professionals (physicians and nurse practitioners) to manage referrals to Texas A&M Health Family Care.

Required Education:
Bachelor’s degree or equivalent combination of education and experience.


Required Experience:
Four years of program administration experience.


Preferred Education:
Bachelor’s degree or equivalent combination of education and experience, and some graduate-level education in a related field.


Preferred Experience:
More than four years of program administration experience.

Experience in a clinical setting, specifically with cancer screening, colonoscopies, lung screening, or
women’s health procedures.


Required Licenses and Certifications:
TX Driver’s License or ability to obtain one within 30 days of employment.


Preferred Licenses and Certifications:
Department of State Health Services (DSHS) certification, certified Community Health Worker (CHW), or able to successfully complete the CHW certification within six months of employment.


Required Special Knowledge, Skills, and Abilities:
Ability to speak, read, write English and Spanish.
Knowledge of word processing, spreadsheet, and database applications.
Knowledge of health care education.
Ability to multi-task and work cooperatively with others.
Ability to work with sensitive information and maintain confidentiality.
Strong interpersonal and organizational skills.
Strong verbal and written communication skills.
Experience working with diverse individuals and communities.

Job Responsibilities:

Documentation and Data Collection
Manages documents to meet patient care needs and grant requirements, including medical records, specialty consultation referrals, and financial assistance forms and other and grant related documents.

Assists in coordination and communication of laboratory and radiology reports.

Accurately maintains daily activity logs regarding client activity and destinations.

Collects data on clinical services provided to diverse populations and barriers to healthcare access.


Patient Coordination and Clinical Support
Counsels individuals, families, and/or groups and engages in health promotion and disease prevention programs.

Coordinates appointments, ensuring that appropriate medical records are available.

Facilitates and coordinates services between health care providers.

Communicates with patients, serving as a liaison between clinic physicians, staff, patients, and outside providers.

Assists clinical support staff in the flow of patients within the clinical environment.


Patient Outreach and Recruitment
Participates in patient recruitment, program planning, implementation, and evaluation.

Plans, organizes, drives to and participates in community outreach events, health fairs, and workshops, and county-based cancer screening events.

Focuses efforts on diverse populations, utilizing bilingual communication skills.

Provides assistance to patients seeking financial, healthcare, and other social and/or community support services.

Continuously expands knowledge and understanding of community resources and service programs provided.

Coordinates with healthcare professionals for promotion and scheduling of grant clinical services at Texas A&M Health Family Care to support and expand programs.


Mobile Mammogram and Well Women’s Day Program Coordination
Coordinates logistics, staffing, and scheduling for grant program offering free mobile mammography.

Coordinates logistics, staffing, supplies, and scheduling with partnering clinics for well-women’s day events.


Other Duties
Performs other duties as assigned.
