Salary: $18.20/ hour
Other Requirements and Factors: Ability to work beyond normal office hours and/or work on weekends.
Required Education and Experience:
Bachelor’s degree or equivalent combination of education and experience
Required Licenses and Certifications:
Texas Department of State Health Services Community Health Worker certification or ability to obtain certification within six months of employment
Required Knowledge, Skills, and Abilities:
Knowledge of word processing and spreadsheet applications. Strong written and verbal communication skills. Excellent analytical, interpersonal, and organizational skills. Ability to multitask and work cooperatively with others
Preferred Qualifications:
Bilingual (English and Spanish)
Certified Medical Assistant
Two years of experience in public health and/or health outreach and education
Experience in leading and conducting specialized training
Experience working/ interacting with health care agencies and providers
Responsibilities:
Program Assistance - Recommends changes to the Wellness in Motion Program and may assist in updating policies and procedures. Continuously expands knowledge and understanding of community resources, services, and programs. Provides assistance to the fitness coordinator in the education and coordination of the Wellness Program. Conducts group and individual training sessions, including correction of exercise technique and spotting participants. Assists participants in reviewing programs and results in an effective manner while strictly adhering to the policies and procedures in place; motivates participants to be active and engaged participants in their health; builds and maintains positive working relationships with the participants, healthcare providers and community organizations. Travels to various locations for program activities. Assists in coordinating logistics and travel for program seminars, workshops, and other special events. Performs other duties as assigned.
Data and Files - Creates, establishes and maintains program files, databases, correspondence and program materials. Assists with initial and follow-up assessments, including biometric testing of blood sugar, HbA1c, cholesterol, triglycerides, blood pressure, body composition testing and fitness testing/evaluation using appropriate methods and equipment. Prepares program correspondence. Assists with documentation, evaluation, accurate reporting, and discussion of results with participants.
Scheduling - Schedules Wellness Program activities, including initial participant assessments, follow-up assessments, and training sessions. Assists in maintaining appropriate documentation of scheduled events and communicating pertinent information with program leadership.
Program Files & Recruitment - Serves as first point of contact for program information and coordinates responses to requests for program information. Assists with Wellness Program promotion and participant recruitment, including participation at community events and health fairs, identifying potential community partners, distributing marketing materials, and completion of registration procedures for new program participants.
Fitness Activities - Assists in identifying alternative fitness activities within the community for participant involvement by engaging with current and prospective community partners. Identifies and refers participants to other beneficial Healthy South Texas programs.
