Position Summary | The UNC Center for Excellence in Community Mental Health seeks a full-time Professional Nurse (RN) to join the Assertive Community Treatment Team (ACTT) serving Wake County. ACTT consists of professionals from multiple disciplines (psychiatry, social work, nursing, rehabilitation counseling, and others) who use a team approach to meet the needs of individuals with severe, persistent mental illness in community-based settings. ACTT works according to a recovery-based philosophy and provides person-centered services addressing the breadth of an individual’s needs, helping them to achieve personal goals. ACTT is generally the first-responder and single point of responsibility for meeting the client’s needs, which necessitates a higher frequency and intensity of services and a low staff to client ratio. Under the direction of the ACTT psychiatrist and Team Lead, the nurse carries out a range of functions on the team, including administration of long-acting injectable medications, oversight of the medication management system, assessment and monitoring of physical health issues, and coordination with other medical professionals and pharmacy providers. This position participates in the ACT on-call rotation. The primary purpose of this position is to provide nursing functions for patients served by an assertive community treatment team. Under the direction of the team psychiatrist, the nurse will provide management and supervision of medication administration to patients in their homes and other settings. Will provide case management services for an assigned group of patients, including planning, developing, and monitoring of services and supports as delineated in the patient’s treatment plan. |
Minimum Education and Experience Requirements | License to practice as a Registered Nurse in the State of North Carolina. |
Required Qualifications, Competencies, and Experience | Knowledge of basic nursing principles and techniques; ability to provide patients and family information on services provided by the ACT team and the larger Center for Excellence in Community Mental Health; ability to independently implement the nursing process to meet patient needs; ability to deliver services in an organized, timely and methodical method; ability to communicate information to patient, families and staff in a concise and descriptive manner, verbally, in writing or by demonstration, using layman’s terms and within HIPAA guidelines. |
Preferred Qualifications, Competencies, and Experience | Previous experience working with adults with mental illness is preferred. Previous experience in ACTT or other community-based services in preferred. |
