The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary

Columbia Primary Care at Columbia University Irving Medical Center (CUIMC) is seeking a Primary Care Nurse III (RN) to join our team. The Primary Care Nurse III will report directly to the Practice Manager or Director of Nursing, Columbia Primary Care. This ambulatory nursing position will serve as a key member of the patient care team to support the pediatric and/or adult populations we serve. The Primary Care Nurse III performs nursing duties consistent with the NYS nursing scope of practice related to the support of the day-to-day clinical operations of the assigned practice. These duties include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, oversight of the practice(s)' infection prevention and control procedures, and patient education that can be in-person, via our patient portal, or by phone. This experienced RN will perform telephonic, electronic, and in-person nursing assessments consistent with the NYS nursing scope of practice. The Primary Care Nurse III will participate in or lead population health initiatives within Columbia Primary Care and the wider ColumbiaDoctors organization to address healthcare gaps and promote positive health outcomes for patients. At this senior level, the Primary Care Nurse III may be asked to travel to all Columbia Primary Care sites to train staff or review processes with staff, providers, and managers. This position will work closely with multiple providers as well as other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture of kindness, collaboration, integrity, accountability, and excellence.


Responsibilities

Administrative/Operations

  • Executes nursing interventions by collaborating with the care team and following the standards of care established by Columbia Primary Care
  • Assesses and systematically collects data related to patient health status and works with the care team to develop the plan of care
    Utilizes data to continuously meet care coordination needs across settings in conjunction with the patient, family, and/or other care providers
  • Assumes responsibility and accountability for effectively managing the nursing care of individual patients
  • Documents nursing assessment and interventions to reflect a comprehensive and integrated approach to patient care
  • Identifies learning needs of the patient and families, educates by adapting to patient and family learning style utilizing the teach-back method
  • Provides care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based
  • There are skills and responsibilities that are common across Columbia Primary Care. The Registered Nurse (RN) may perform any or all of these skills and responsibilities:
    • Telephone, My Chart, and in-person triage (for symptom management)
    • Review and act on test results (lab, x-ray)
    • Review and manage communications from internal and external ???customers??? that require the education and training of an RN
    • Providing patient education through multiple avenues inclusive of in-person, by telephone, or video
    • Execute delegation protocols
    • Assist with medication management (e.g. medication renewals)
    • Complete accurate and timely documentation in the electronic medical record
    • Assist with the management of urgent and emergent patient care situations
    • Participate in care coordination and population health activities in collaboration with the central teams and related to any current initiatives within Columbia Primary Care.
    • Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family, and/or other care providers
    • Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness
    • Establish standard clinical supply storage in collaboration with other clinical staff and audit that standards are being maintained. Establish standard protocols for medication/immunization inventory management and storage
  • May float to other clinics as needed for operations needs.
  • Serve on relevant committees, task forces, and workgroups within Columbia Primary Care and the Organization
  • Other duties as assigned appropriate to scope, education, and/or training

People

  • Participate in the recruitment of clinical staff
  • Precept clinical staff (medical assistants, licensed practical nurses, and RNs and perform competency testing for new and established clinical staff.
  • Serve as a mentor and resource to others

Compliance

  • Maintain patient privacy and confidentiality according to HIPAA requirements at all times.
  • Keep current on all organizational and practice policies (e.g., infection control, HIPAA), goals, initiatives, and required trainings
  • Ensure that University and Department policies and procedures are followed.
  • Successfully complete all required university, hospital, and department trainings.
  • Other duties as assigned within the professional scope of practice/training/education.

Minimum Qualifications

  • Requires a bachelor's degree in Nursing or equivalent in education, training, and experience, plus four years of related experience.
  • An equivalent combination of education, training, and experience may be considered.
  • Current and unrestricted New York State RN license.
  • BLS certified.
  • Ability to work indoors in an environmentally-controlled environment with a degree of dust, noise, odors, etc., without undue discomfort.
  • Ability to perform sedentary work that primarily involves sitting or standing for long periods.
  • Travel between clinical sites may be required depending on operational need.
  • Duties performed routinely require exposure to blood, bodily fluid, and tissue. The incumbent is expected to perform and/or/assist in emergency care and/or first aid as needed.
  • Ability to work collaboratively with a culturally diverse staff and patient/family population demonstrating tact, respect, and sensitivity.
  • Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management, and other stakeholders. Must have highly developed interpersonal skills and the ability to de-escalate stressful situations.
  • Must be an engaged and positive team member demonstrating collaboration, inclusion, reliability, punctuality, and adaptability to new and changing situations including helping peers adapt to new situations.
  • Strong proficiency in computer skills including, but not limited to, Electronic Health Systems, Microsoft Excel, Microsoft Word, Email systems, etc.
  • Must be a motivated individual with a positive mindset and exceptional work ethic.
  • Must successfully complete systems training requirements.
