Post Acute Care Clinical Liaison

The Post Acute Care Liaison conducts patient assessments to identify patients for potential admission into post acute affiliates and partners. Assigned facilities and geographical territories are assigned for managing patient progression that are generated through operations. Creatively utilizes educational opportunities about the health system to present the system to intake sources. Develops and manages marketing strategies for assigned locations and travels to locations within the service area. Generates sources for potential patients to transfer to from the hospital while serving as a liaison for our system, affiliated agencies, and providers.

  • Prospecting methods are used to generate leads and capture new capacity within the post acute space
  • Manage the medical center discharge relationships as an ambassador for patients who have had recent hospitalization
  • Participates in ongoing marketing planning activities and strategies
  • Maintains a positive and professional work environment.
  • Investigates documents and resolves problems and complaints.
  • In addition to the above job responsibilities, other duties may be assigned.

This position requires one of the following accredited degrees and/or certifications - Registered Nurse (RN), Respiratory Therapy (RT), or Bachelors of Social Work (BSW).

Position Compensation Range: $61,443.20 - $98,300.80 Annual

MINIMUM REQUIREMENTS

Education:  One of the following depending on specialty:

RT: Graduate of an accredited Respiratory Therapy program required.

Marketing:  Bachelor’s Degree required. 

Experience: 2 years relevant experience 

Licensure: One of the following depending on specialty:

RT:  Certified Respiratory Therapist (CRT) with the National Board for Respiratory Care (NBRC).  Certified to practice as a Respiratory Care Practitioner in the Commonwealth of Virginia. Registered Respiratory Therapist (RRT) with the National Board for Respiratory Care (NBRC) required.  Valid Virginia Driver’s License (VADL) required.  Marketing:  Valid Virginia Driver’s License (VADL) required. 

PHYSICAL DEMANDS

Job requires Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly.
