The Sylvester Comprehensive Cancer Center is currently seeking a Pharmacy Practice Resident. The incumbent in this position will develop advanced skills in the areas of patient care, drug information and pharmacy administration. Build upon previous education and experience directed toward developing expert knowledge and skills in pharmacotherapy management by working closely with healthcare practitioners. This is an educational hands-on experience that also provides a staffing component within the department.

Responsible for order entry and dispensing of medications. Provides pharmaceutical care to ensure safe, cost effective, rational drug therapy. Pharmacists, being exempt personnel, will need to do additional tasks and duties that will exceed their shift requirements.

Provides and coordinates patient-oriented pharmaceutical care services including therapeutic drug monitoring, pharmacokinetic drug dosing, drug regimen review and drug use evaluations in all patient care areas in a variety of practice settings throughout the residency. Provides appropriate training and educational support for pharmacy staff as well as physicians and other professional ancillary staff, patients and their caregivers regarding safe and effective drug therapy throughout the residency. Assists in developing and monitoring of pathways, protocols and mechanisms supporting safe, efficacious and cost-effective use of medications. Assists with distributive functions on a regular basis throughout residency program which includes holiday and weekend assignments.

JOB RESPONSIBILIITES

The pharmacy practice residency is a twelve-month post-graduate program designed to provide the resident with extensive, broad-based training and experience in pharmacy practice.

The resident actively participates in achieving the educational goals and objectives for each training experience. Participates in the design and completion of a residency project.

Interprets and processes physicians' drug orders and provides information to medical, nursing and other healthcare personnel concerning the optimal, safe use of medication therapy. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectfulto all.

  • Participates in training and educational activities of the residency program to produce the following outcomes:

            a) acquire the knowledge necessary for high-level problem solving

            b) refine problem solving strategies

            c) strengthen professional and leadership values and activities

            d) advance growth in clinical judgment

  • Displays initiative in providing optimal patient medication therapy through clinical pharmacy services, outcome measures, education, special projects, innovative services, and other appropriate activities.

  • Interprets and processes physician orders, monitors drug interactions, allergies and drug incompatibilities, and dispenses and performs the final check of all medications provided for patient use.

  • Reviews copy of physician’s original orders for accuracy and appropriateness of therapy in terms of dose, route, directions for use and possible therapeutic incompatibilities (i.e., allergy, duplication of therapy, drug interactions, dosing adjustments based on creatinine clearance, age/weight and I.V. to PO when appropriate) to ensure safe and appropriate drug therapy.

  • Maintains active involvement in local, state, and/or national pharmacy professional organizations.

  • Formulates a system for staying current with pertinent pharmacy, medical, and scientific literature.

  • Maintains knowledge and understanding of Pharmacy and Medical Center information systems.

  • Delivers and documents effective education and training using appropriate techniques.

  • Presents pharmacy concerns, solutions, and interests in a balanced and effective manner.

  • Effectively uses time-management skills to fulfill practice and training responsibilities.

  • Participates in the organization's formulary process.

  • Communicates with physicians to clarify any discrepancies and prevent misadventures, promotes rational drug therapy by recommending formulary therapeutic alternatives when a nonformulary drug is requested and records interventions in appropriate manner, including physician’s name. Seeks reasons why medical necessity for non-formulary medications and documents such reasons.

  • Provides pharmacokinetic support on indicated medications.

  • Reports to department head before, during and after any disaster situation as instructed by such for updates and/or changes in work schedules as may be determined necessary.

POSITION REQUIREMENTS:

            Education/Credentials/License and Certifications

            Formal Education (Check one level only):

            _____ High School Diploma preferred

            _____ Two Year College Degree

            __X__ Four Year College Degree – in Pharmacy

            _____ Graduate Degree

            Certification/Licensure:

  • Current Florida State Pharmacist License – To be obtained upon initiation

            Preferred Registration, Licensure or Certification:

  • Current Basic Life Support (BLS) certification

  • Current Advanced Cardiac Life Support (ACLS) certification
