GeneralDescrip tion of the Role

Meets clinical requirements of ARCP. Plans, directs and participates inthe delivery of patient care. Coordinates the plan of care for patients with complexrespiratory disord ers from acute care to outpatient management. Acts as aliaison betweenth e patient and healthcare personnel to ensure necessary careand equipment is provided promptly and effectively. Responsibilities includeidentifyi ng and educating on health risks, monitoring patient compliance withcurr ent disease specific clinical guidelines, coordinating with outpatientcl inics, educating patients and their families on their specific diseasepr ocess, and providing ongoing assessment, monitoring and re-evaluationofn eeds. Provides support to inpatient RCS team with education and policy r eviewwith evidence based standards of care.< /strong>

·        Workcollabor atively with physicians, nursing, and other members of themulti-discipli nary team providing targeted management and education to patientsdiagnos ed with a complex respiratory disorder. Develops individual action plans that help coordinate care, manage costs, and open lines of communication .

·        Nav igatethe patients care to include contacting appropriate team members/ag encies toassist in the determination of patients’ needs and/or eligibili ty for services.

·        < /span>Monitorsprocedural/clinic schedules to ensure appropriate ne eds and follow up calls areinitiated and patients are aware of what they should expect for inpatient ordischarge processes.

< span>·        Participatesin quality in itiatives to drive improved patient outcomes.

·         Helpsensure service growth by p articipation in outreach strategies such as healthfairs, screenings, and community education.

·        </s pan>Completesprojects in assigned timeline. Analyzesproject data for departmental processimprovement. Demonstrates abilityto priori tize and efficiently manageassignments.

    • Routine care: </l i>
      • Routine patient care will be provided bythe a ssigned RCP unless a specific assessment is required, at which point the Navigator will communicate with the assigned RCP.</s pan>
      • Weekly rounding with care team and Navigator to review patient progress with plan of care and communication with assigned RCP following.
      • Tracheos tomy discharge education and device education will be overseen by Navigator. Initial education and Emergency scenarios will be condu cted by Navigator or specific team. All other classes can be taugh t be assigned RCP.

RequiredQualifications at this Level

Education

Completion of a 2 year AMA approvedrespiratory care program (Associates Degree or the equivalen t) and NorthCarolina State Licensure and Registry (RRT) by the National Association forRespiratory Care (NBRC).  

Must maintain annual skill maintenancerequirements

Must maintain annual safety/compliancerequirements

Must maintain North Carolina annual licenserequirements for CEUs

Experience

Level I Trau ma - ICU experience stronglypreferred

Twoyears of cli nical experience followinggraduation from a respiratory careprogram</spa n>

Certification in Basic Life Support (BLS) andAdvanced Car diac Life Support (ACLS)

Pediatric RCP’s need your N RP, PALS andACLS within 90 days of hire.

Know ledge,Skills, and Abilities

  • Knowledge necessary to make appropriate clinical decisions and interaction rel ative to the specific age of a patient.
  • Skills validation/certification of competencies for the duties and res ponsibilities required for a Respiratory Care Practitioner.

Minimum Qualifications
Education

Experience

Six months to one year of clinical experience following graduation from a respiratory care program.
