Job Summary:

The Patient Navigator will be working primarily on the Texas Healthy Women Program for outreach, recruitment, application assistance and referrals.

 

Major Responsibilities / Duties / Critical Tasks:

  • Plans and delivers community outreach and in reach activities to educate the community about the importance of women health programs.
  • Provides patient navigation services to facilitate access and assist with scheduling.
  • Participates in the development of program timelines and documents progress towards meeting program milestones, metrics, and deliverables.
  • Supports a cohesive team effort to accomplish approved goals, milestones, and metrics.
  • Documents accurate and complete data on outreach and patient navigation services provided. 
  • Maintains confidential records /sensitive patient information with discretion and adherence to HIPAA rules.
  • Promotes the UT Tyler HSC mission, vision, and values by action.
  • Responsible for other duties as assigned.

  • Knowledge of community resources and experience working with a variety of organizations is preferred.
  • Knowledge of Spanish (Bilingual) is preferred.


Qualifications

 

Required Education / Experience:

Minimum Education:
High School Diploma or GED.

Minimum Experience:

Three (3) years of relevant experience in customer service, patient navigation, or other community-oriented programs in diverse settings. Basic computer skills required.

 

Preferred Experience: Experience working with diverse stakeholder groups and communities.
May accept a combination of required education and experience.
