The University of Miami is currently seeking a Patient Navigator to work at Sylvester Cancer Center (SCCC) in Miami. The incumbent facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

CORE JOB FUCNTIONS

  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.

  • Ensures patients have access to insurance and social service programs for which they are eligible.

  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.

  • Identifies and resolves real or perceived barriers to care. Educates and counsels patients on issues related to their health and life situations.

  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

DEPARTMENT ADDENDUM

Department Specific Functions

  • Coordinates and schedules care for specific cancer programs (radioactive iodine, cancer supportive services, & survivorship program).

  • Cross covers for the lymphedema program patient navigator and other staff when needed.

  • Assist with incoming referrals and schedules care with physicians and other allied staff input on the cancer patient’s care.

  • Assures all appropriate and standard exams/tests have been scheduled and completed prior to treatment. Reminds providers of test results for review.

  • Collaborates with other departments (inpatient unit, radiology, nuclear medicine, infusion, and others) to coordinate care.

  • Assures needed treatment is ordered on time and delivered for day of treatment. Collaborates with cancer supportive services for adjunct care when needed.

  • Refers patients to advocacy groups and other patient support organizations. Updates data for the program.

  • Troubleshoots insurance and financial concerns with the care team (CIV, utilization review, and CBO) Answers patients calls and communicates with providers questions/concerns that require a clinical response.

  • Manages in-baskets and secure chats via EPIC.

  • Escalates any barriers/difficulties with patient care to appropriate staff/leader.

Education:

High School diploma or equivalent

Experience:

Minimum 3 years of relevant work experience
