The UWMC Urology Clinic has an outstanding opportunity for a Patient Care Coordinator (Full-Time, Day Shifts). The Patient Care Coordinator (PCC) is responsible for coordinating the delivery of medical care, as formulated by UWMC physicians or their designee, and the patient’s access to various health care services. The PCC will coordinate the patient’s entry into and through the health care system, coordinate patient care services and serve as patient advocate.

Under the general direction of the Department Chairperson and his/her designee, with assistance from the Health Service Manager, the PCC will use knowledge of the clinic, division and hospital policies/procedures, insurance coverage requirements including pre-admission authorization, and third party payer and reimbursement procedures to coordinate all patient services such as admitting, arranging diagnostic tests or procedures, scheduling surgery and consultation requests, and scheduling services of other hospital departments to facilitate delivery of care to patients.

Job Duties Include

Patient Access
•Acts as primary liaison for UWMC/UW Medicine and represent departmental physician (s) with the hospital, patient and family, referring physicians, outside agencies, staff from outside hospitals and general public by providing information and advice regarding UWMC hospital services.
•Works independently under the general direction of a physician or their designee.
•Uses ability to triage medical information in a specific medical or surgical specialty.
•Uses effective oral and written communication with a variety of Health Care Professionals, patients and their advocates, hospital and clinic personnel both at UWMC and other medical facilities, representatives of insurance companies and other organizations related to the health care industry.
•Demonstrates the ability to be organized and effectively prioritizes work tasks for the needs of the clinic.
•Requests interpreters as appropriate for appointments, admissions, and surgery.
•Uses a variety of computer systems such as REG/ADT, EPIC, ESI, ORCA/Mindscape, OneHealthPort. RECONDO and Microsoft Products.
•Maintains knowledge of billing and medical record documentation requirements, diagnostic tests and procedures and specialized pre-procedure instructions with guidelines set by the department or clinic Chairperson or Medical Director.

Patient Intake/Insurance
•Answer the telephone in a professional and courteous manner and directs calls appropriately.
•Responsible for reviewing registration data such as demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done.
•May request a quote or estimate for surgeries to patients that have been seen and scheduled for an OR procedure.  May provide estimated charges based on plan of care for purpose of patient deposits when patient is determined to be self-pay.  This is done in conjunction with Financial Services and under the direction of the Health Services Manager, clinic Manager/Department Administrator.
•Contacts review organizations and insurance companies to ensure prior approval requirements are met before patients enter the system such as pre-authorization and referrals; present necessary medical information such as history, diagnosis, prognosis, and length of stay prior to admissions; provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.

Coordination:
•Acts as a liaison between patients, consulting physician, referring physician or outside agency to coordinate patient appointments, admission, or surgery.  Will coordinate the exchange of information either written or verbal before and after patients are seen.

Scheduling: Admission
•Responsible for reviewing registration data with patient for scheduled inpatient surgery/admission or outpatient surgery/procedures. E.g. demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done.
•Responsible for coordination of pre-admission tests and procedures.
•Coordinates the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff and admitting through the Patient Flow supervisor.
•Responsible to communicate any cancellations or changes to Admitting, Financial Clearance Center, UWP, Financial Counseling and medical staff.

Scheduling: Surgery
•Acts as liaison between patient, physicians, OR, nurses, other clinics, nursing units, admitting, Financial Counseling, blood bank, lab, Radiology, OT, PT, Prosthetics, etc. for the purpose of coordinating all pre-op planning and preparation.
•Schedules and communicates pre-op planning and preparation to patient. Sends appointment and other information to patients.
•Responsible for requesting necessary surgical equipment, tools, supplies, devices, and implants (will supply necessary info for custom implants).
•Manages OR block time. Releases block time and requests elective OR time through the Surgical Services Website.
•Responsible for scheduling, rescheduling, and canceling surgeries on ESI scheduling system. Will monitor case order day prior to surgery and make any necessary changes.
•May assist with completion of surgery packet.
•Follow-up Care:
•May assist with discharge from clinic or hospital by providing follow-up appointments, arranging transportation or housing.

Desired:
Candidates with previous Patient Care Coordinator experience desired.

Requirements:
A Bachelor's Degree in a medically related or public relations field AND two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology OR equivalent education/experience.
