Job Summary

The UCSF Pediatric and Adolescent Gynecology (PAG) Clinic is a specialty clinic that started in 2021. During this time the unique nature of these patients, the legal limitations of their emancipated reproductive care and their psycho-social needs have all been under-served. This prompted the application for a patient navigator who can support patients through their scheduling, clinical care navigation and very importantly their referrals for follow up specialty care. The patient navigator will reach out to patients before their appointments and insure that contact information and confidentiality is maintained as patient wishes it to be. They will be guided through the proxy process with EPIC so that their parents can have access to their chart if they would like this.  Their appointment times and locations will be confirmed. There will also be discussion of what their needs are for their visit to allow for some anticipatory guidance and planing for follow up. If the patient is in need of psychological or social support they will be put in touch with resources to meet these needs. Finally, they will offer after visit care and follow up for patients to ensure they have completed proper testing, radiology studies and were able to schedule referral appointments as well as follow up appointments.

The advocate acts as a communication liaison to understand the patient’s needs and concerns and supports the patient in advocating for their own needs in their appointments. This involves attending patient’s appointments in-person or virtually as requested by the patient and triaged by acuity, type of appointment and patient need. This position will track common challenges, concerns or issues for patients and their families and provide summary data for clinic managers and leadership.

The patient advocate must be able to work in a fast-paced environment, independently analyze issues, work with minimal supervision, and be exceedingly well-organized and flexible. Diplomacy and tact are essential, together with excellent verbal and written communication skills.

This position seeks candidates who have experience advocating for patients and a commitment to fostering anti-ageist practice, social and reproductive justice, and health equity, who will help create more accessible, equitable, coordinated, culturally responsive and person-centered care.

The final salary and offer components are subject to additional approvals based on UC policy.
