JOB DESCRIPTION AND POSITION REQUIREMENTS:

The Office of Cancer Health Equity, Penn State Cancer Institute, in collaboration with the Department of Family and Community Medicine, is seeking to fill a part-time, clinical-community health worker position.  The person in this position will coordinate care and serve as intermediary between patients and clinicians for cancer prevention and early detection.  The person in this position will demonstrate knowledge and experience in cancer risk reduction and clinical care.  The person in this position will demonstrate attention-to-detail by maintaining accurate records of patient encounters.  The person in this position will be and effective communicator, both in written and verbal format, and navigator of patients.  To be considered, candidate should have experience in a clinical environment, such as being a nurse, social worker, or care coordinator, as well as experience in patient education and navigation.

Education and Experience: 5+ years of related experience; bachelor’s degree or higher

Work duties:

  • Conduct assessment of patient barriers to cancer-related care
  • Use motivational interviewing and other counseling behaviors to navigate patients
  • Identify and coordinate community services for patients
  • Review medical records to assess patient health status
  • Keep detailed notes and records of patient encounters
  • Participate in research studies by recruiting or navigating participants
  • Contribute to the clinic-community cancer health worker program
  • Maintain secure health records and data on referred patients, services and outcomes
  • Demonstrate cultural sensitivity in delivery of care
  • Collaborate with team members to provide health care services
  • Collaborate with team members to implement strategies to reduce cancer burden
  • Supervise the work of assigned personnel and students

Pursuant to the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act and the Pennsylvania Act of 1988, Penn State publishes a combined Annual Security and Annual Fire Safety Report (ASR). The ASR includes crime statistics and institutional policies concerning campus security, such as those concerning alcohol and drug use, crime prevention, the reporting of crimes, sexual assault, and other matters.
