UW Primary Care (UWPC) is a network of 16-Community-based clinics throughout the Puget Sound region. Our clinics offer a wide spectrum of primary care services, as well as secondary care, from pediatrics to geriatrics. Urgent care services are offered seven days a week at select locations. UW Medicine's Primary Care clinics are consistently recognized for high quality standards by the Accreditation Association of Ambulatory Health Care and the National Committee for Quality Assurance Patient-Centered Medical Home Program.

This category recognizes outstanding achievement and providing ongoing excellence through effective collaboration.

UW Primary Care/Population Health - Federal Way has an outstanding opportunity for a Full-time PANEL NAVIGATOR.

The Panel Navigator is a specialized position within our family of Patient Services Specialists that champions population health activities that are critical to our patient's experience and care outcomes. Panel Navigators enhance the patient experience through proactive engagement and outreach around wellness and preventative care, doing so with continual focus on improving the “global” patient experience.  

The Panel Navigator works directly with our Federal Way Providers and the clinic's care team to ensure an informed and connected continuum of care for our patients. 

Population Health advances our community's health by managing accurate patient records, ensuring team-based panel management, and initiating outreach to patients for preventative screening. Panel Navigators refer patients for care coordination, care management, and address disparities in care for vulnerable populations to ensuring our patients’ needs are met.
