The UW MEDICAL CENTER - MONTLAKE - SOCIAL WORK DEPARTMENT has an outstanding opportunity for a PART-TIME OUTPATIENT SOCIAL WORKER (80% FTE, Day Shifts). The Social Worker will work on triage of incoming referrals and appropriate responses, which may include; crisis intervention, psychosocial assessment, resource coordination, consultation to interdisciplinary staff, caregiver support, end-of-life and care need transitions, pre-surgery transportation/lodging assistance and other support for patients with complex needs.  This position provides opportunities to work in a fast-paced, challenging, diverse and rewarding environment.

This position will support patients and caregivers as part of the ALS Interdisciplinary clinic team on Wednesdays (8:30-5:00 pm)  and a variety of UWMC Montlake Outpatient Clinics four days a week (M/T/Th/F – 9:30-4:00 pm).

PRIMARY RESPONSIBILITIES:
• Provide distress screening follow-up on referred or screened clinic patients to determine the need for social work intervention.
• Initiate appropriate interventions that may include assessment, financial need, housing, legal issues, drug/alcohol issues, mental health needs, coping and adjustment to illness, advance care and end of life planning, coordination with community resources, coordination with Medicaid brokers and support systems, and address other psychosocial and tangible needs that could affect participation in care.
• Provide patient and caregiver education on a wide range of resources and community services applicable to the patient’s care needs and assisting with referrals where appropriate.
• Create and maintain resource and referral information for each domain of the oncology distress screening tool that may be utilized by multidisciplinary staff for responses not meeting criteria for SW intervention.

REQUIREMENTS:
A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENT:
Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
• Agency Affiliated Counselor
• Licensed Social Work Associate -Advanced
• Licensed Social Work Associate - Independent Clinical
• Licensed Advance Social Worker
• Licensed Independent Clinical Social Worker
