Job Summary

This Nurse Practitioner (NP) position is a position with a primary focus on the coordination of the UCSF Medical Center inpatient hospice program that was launched in March 2021. Within this role, the NP will systematically identify patients nearing the end of life to determine eligibility into the inpatient hospice program and provide counseling and education to patients and families considering enrollment. The NP will then serve as the primary provider for enrolled patients, working closely with an interdisciplinary team of hospice clinicians (nurse case managers, social worker and chaplain), a dedicated hospice attending physician, and an on-site palliative care physician to manage symptoms and provide support to patients and families. They will also participate in ongoing program development for the inpatient hospice program through data collection, staff/provider education, reporting, and program evaluation.

In addition to their responsibilities to the inpatient hospice program, the NP will also serve as a resource for all staff and providers caring for hospitalized patients at end of life – regardless of whether they are participating in the inpatient hospice program or are followed by the Palliative Care Service. When time allows, the NP may assist the palliative care consult teams in triaging new consult requests and in independently providing palliative care consultations in collaboration with the larger trans-disciplinary team. 

In collaboration with the attending physicians on service and in partnership with the other members of the trans-disciplinary palliative care team, the NP will work at the top of their license and round on patients independently when indicated. The NP should have in-depth knowledge and clinical expertise in serious illness communication and symptom management. The NP will assist patients and families with medical decision making; help teams clarify values and goals of care; facilitate emotional, spiritual, and bereavement support; and manage symptoms like pain, nausea, anxiety, dyspnea, constipation, and insomnia. In addition, the NP will have continued responsibilities for patient, family, and staff education; clinical consultation; discharge coordination; research; and departmental leadership.

We are seeking a clinically outstanding and compassionate NP with excellent communication skills who is able to continue to support the success of the inpatient hospice program while providing support to all patients nearing the end of life and to the staff and providers caring for those patients.

UCSF Medical Center requires all Advanced Practice Providers (APP) to be credentialed through its Office of Medical Affairs & Governances (OMAG) in order to practice and function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings at all UCSF Medical Center sites (Parnassus, Mt. Zion, Satellite Clinics, et al). Credentialing and approval of the Standardized Procedures takes between 60-90 days and must be completed prior to the first working day.

Required Qualifications

  • Minimum of 2 years’ experience working in a clinical capacity in an acute care hospital setting
  • Ability to provide compassionate care and support both for patients and their families, as well as the other clinicians involved with the patient’s care
  • Excellent communication skills, professionalism, and an interest in continuing to help the inpatient hospice service develop strong relationships with other primary teams in the hospital
  • Clinical expertise with symptom management at end of life
  • Familiarity with Medicare Hospice regulations

Preferred Qualifications

  • A year or more of inpatient NP experience in an acute care hospital setting
  • Certification through the National Board for Certification of Hospice and Palliative Care Nurses (CHPN) as either Advanced Certified Hospice and Palliative Care Nurse (ACHPN) or Certified Hospice and Palliative Care Nurse (CHPN)

License/Certification

  • California driver’s license (CDL) if required for position.   
  • Must hold current national certification from an accepted certifying agency and provide by first day of work.
  • Must hold a current and active RN and NP and Furnishing license in California.
  • Must have completed a BRN recognized graduate master’s degree program as a nurse practitioner.
  • Must obtain an active and/or transferable DEA license to California and provide evidence by 90th day of clinical practice. (It is the responsibility of the employee to transfer their DEA license to their new place of employment if from out of state, via online DEA portal and provide evidence of transfer).
  • Must have submitted enrollment with CMS Medicare and Medicaid through credentialing by first clinical day.  CCS enrollment as appropriate for pediatric environments.
  • Must hold current AHA BLS as minimum requirement        
  • State regulation requires that you have a supervising physician to practice
  • Incumbents with less than 6 months of NP experience will be hired as a Nurse Practitioner 1
UCSF Health requires all Advanced Health Practitioners (APP) to be credentialed through OMAG to practice and be privileged through CIDP to function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings at all UCSF Health sites and affiliates. Credentialing, health plan enrollment, and approval of privileges must be completed prior to the first working day. Inability to comply with the requirements of OMAG/CIDP AT ALL TIMES will result in either, a LOA or suspension of privileges designation.
