Survivorship Advanced Practice Provider

Nurse Practitioner or Physician Assistant

Job Location

The APPs associated with the Survivorship Program will practice at several locations as needed (DCI Cancer Center (Durham), Women’s Cancer Center (Macon Pond, Raleigh), Duke Primary Care Sites, Duke Family Medicine Center and/or telehealth.)

After a period of on-site training, this role will be a hybrid position, with 1-2 days per week working remotely.

Job Summary    

Survivorship Patients

Provides direct patient care in outpatient settings. Create personalized care plans to identify and treat the long-term effects of cancer treatment and address psychosocial concerns.  These plans include screening regimens to watch for the return of cancer or the development of other cancers. The age-specific scope of practice is between 18 yrs. through adulthood.

Elevated PSA Patients

Provide initial assessment and diagnostic decision-making for suspected prostate cancer referrals. Follow a protocol-driven assessment model.

Job Duties and Responsibilities:

  • Take patient histories, conduct physical examinations, and document care as required.
  • Create cancer survivorship care plans for new patients and discuss long-term care with them.
  • Communicate with the patients’ primary care providers in a shared care model.
  • Review all labs, pathology and radiology reports and communicate relevant findings to supervising physician and to patients.
  • Respond to triage nurse calls and all other InBasket inquiries / requests.
  • Interact with scheduling staff to adjust clinic schedule as necessary to assure efficient clinic operations; this includes responding to and managing overbook requests.
  • Order and schedule laboratory studies and screening tests (e.g., echocardiograms).
  • Complete (or delegate where appropriate) forms (such as FMLA and disability, letters of medical necessity, peer to peer reviews) where permitted.  Coordinate supervising physician signing of home health orders.
  • Clinical assignments are to hold 8 clinical sessions per week. A session is typically defined as 4 hours of booked clinical time.
  • You may also have other responsibilities as a member of the Survivorship team consistent with your position as a clinical provider.

Job Eligibility Requirements

Education          

NP:         Graduate of MSN or DNP accredited Nurse Practitioner Program

PA:         Graduate of ARC accredited Master’s Degree or Doctoral Degree Physician Assistant Program

Certification

NP:         Any NP certification that treats adults (FNP, ANP, AGNP, AGACNP, etc.)

PA:         PA-C

Licensure

NP:         Must have unrestricted RN licensure issued by or recognized by the NCBON with authorization to practice as a NP

PA:         Must have unrestricted license issued by NCMB with authorization to practice as PA

Both:     Federal and State DEA independent license with prescriptive authority

Experience

APP experience is preferred, ideally in Family Practice and/or Oncology, but recent or new grads graduating by August 2024 are welcome to apply.

Job Hours

8:00 am – 5:00 pm, Monday-Friday. No on-call, holiday, evening, or weekend coverage is required.
