Position Description:
Advanced Practice Provider - Hematology/Oncology Clinic at Cooper Green Mercy Health Services Authority

The following APPs may be considered for this role:  Family NP, Adult Primary Care NP, or Adult Acute Care NP.

CME allowance: Full-time benefited APPs receive an annual $2,000.00 CME allowance. This can be used for recertification, applicable fees, CME courses, etc. that occur after a new employee's start date.

The Advanced Practice Provider will provide medical care and treatment within established scope of nursing practice to patients as assigned by and under the direction of the collaborating physician (Section 610-X-9 of the Alabama Board of Nursing Administrative Code). The collaborating physician shall be available for direct communication for consultation and referral or arrange for the same medical coverage by a physician who is pre-approved by the State Board of Medical Examiners in the same manner as the collaborating physician of record. This position will coordinate plan of care, discharge, orders, and referrals as appropriate to meet the age specific needs of the patient; plan and coordinate health care education for patients; and perform comprehensive physical examinations and assessments. This position will interpret, analyze, and evaluate patient’s medical record and social data; perform and/or assist the physician with procedures per collaborative agreement; and coordinate and participate in performance improvement activities.

KEY RESPONSIBILITIES:
• Provide thorough physical and psychosocial evaluation for patients:
o Perform complete, detailed and accurate health histories
o Evaluate physical, mental, and psychosocial domains
o Review patient records, reconciles medications, laboratory and other diagnostic testing
o Evaluate patient’s support system’s ability to provide the prescribed therapy
o Accurately evaluate patient’s changing condition and response to interventions

• Interpret and analyze patient data to determine patient status, patient management and treatment:
o Formulate medical and nursing diagnoses based on assessment findings
o Plan and initiate a therapeutic regimen appropriate for complaint, age, race, sex and physical condition of the patient which may include:
 Ordering legend drugs within the approved formulary
 Coordinating and sourcing medical devices
 Provide appropriate timely referrals, nutrition and supportive services, when indicated
 Orders laboratory, radiological and diagnostic studies appropriately
o Assist physician per collaborative practice agreement
o Perform or assist with laboratory procedures and technical procedures
o Recognize the urgency of appropriate situations and responds with immediate physician notification (Critical Lab) and patient plan
o Provide appropriate patient education related to procedures, medications, therapeutic goals, health counseling, to patients
o Display a general understanding of the social determinants of health and population health management tenets

• Complete appropriate documentation on all patients:
o Patient summary list completed/updated, when indicated
o Documents patient physical examinations, evaluations, assessments and records pertinent data in appropriate medical record in a timely and efficient manner; adheres to facility policy for completion timelines
o Complete intradisciplinary education sheets

• Provide ongoing evaluation of patients with chronic and acute illnesses; assist physicians, nursing staff and ancillary staff to ensure continuity of patient care:
o Accept referral from primary care physicians to monitor patients’ ongoing response to therapy
o Collaborate with physicians regarding therapeutic changes when indicated
o Formulate medical and nursing diagnoses and institutes therapy or referrals of patients to the appropriate health care facilities, agencies, social services, home care, physician support or other resources of the community as indicated

• Consistently demonstrate personal accountability for professional growth:
o Meet and maintain current certification/licensure and continuing education requirements without reminders
o Assist Physician/ Clinical Director in Maintaining Current Evaluation
o Maintain appropriate documents for provider status approval (BCBS/ Medicare) and approval with the UABHS Oversight Committee; respond within timelines when documents are requested from facility staff for business purposes
o Maintain knowledge of current issues through attendance or documented reading of at least 80% of staff meetings, as appropriate
o Serve as a role model and clinical resource person for other team members
o Participates in, utilizes, and conducts research and other scholarly activities, as assigned
o Assist in development of patient education materials as appropriate
o Participate in informal education of nursing and support personnel
o Provide structured educational sessions for staff
o As requested, serve in leadership or consultant role on committees and/or projects

• Coordinate and participate in Performance Improvement activities:
o In collaboration with physician; identify, implement and maintain records of the Quality Assurance plan that is in keeping with the rules of the Alabama Board of Nursing and the State of Alabama Board of Medical Examiners
o Participate in peer review as needed
o Display an active role in PI projects of the organization

• Maintain accountability for safe environment for patients and others:
o Promote safety and adheres to safety regulations
o Maintain a neat and organized environment is neat and organized
o Aggressively prepare to institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, allergic reactions and other emergent situations
o Ensure compliance with policies and procedures regarding departmental operations including all regulatory agencies
o Problems are addressed to appropriate department
o Maintain a safe environment appropriate for the patient population
o Report any unsafe and hazardous working conditions to supervisor
o Actively participate as a team member and collaborate with team members on multi-disciplinary approaches
• Respects the dignity, values, and diversity of others
o Maintains confidentiality

• Additional duties (Diagnostic or therapeutic procedures requiring additional training) as requested and credentialed delivered in accordance with the State of Alabama Nursing Board


Position Requirements:
MINIMUM QUALIFICATIONS:
Licensure as a Registered Nurse in Alabama is required for approval and for subsequent renewal of approval. Master’s prepared or higher degree in nursing and graduation from a nurse practitioner educational program required. Certification as a nurse practitioner in the appropriate area of practice from a national certifying body recognized by the Alabama Board of Nursing. Must have an understanding of the impacts on health of and be able to empathize with the most vulnerable and impoverished members of society.

Preferred:  Oncology/Hematology preferred but not required. Must have an understanding of the impacts on health of and be able to empathize with the most vulnerable and impoverished members of society.

CERTIFICATES/LICENSES/REGISTRATIONS ETC:
Currently certified as an Advanced Practice Nurse (APN) with Certified Registered Nurse Practitioner (CRNP) Specialty by the Alabama Board of Nursing required. Certification in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification American Heart Association (AHA) may be required.
