We are seeking a compassionate, dedicated Nurse Practitioner to complete the following job duties with our inpatient/outpatient Advanced Heart Failure and Pre Heart Transplant population.


Essential Duties:
  • Responsible for the planning and coordination of all aspects of the work up and care for the patients in the Advanced Heart Failure and Pre Heart Transplant, throughout the continuum of services - from initial referral to follow up care.
  • Responsible for evaluating the patients (includes: chart review, performing history and physical examinations, ordering diagnostic tests, reviewing results and reporting to the Physician as needed.
  • Provide Inpatient services to Advanced Heart Failure and Pre Heart Transplant patients, coordinate daily activities of inpatient service, including care coordination/patient education, timely documentation of patient progress notes, discharge summaries, and CMS required care coordination notes.
  • Coordinate communication of patient ADT and surgeries with referring and collaborating MDs.
  • Develop and implement patient management plans and assist in provision of continuity of care.
  • Provide education, leadership and patient care management across the health care continuum from preventative and acute care through discharge.
  • Serve as a patient advocate throughout the care process and represent the medical center at regional and national meetings/conferences.

 
Education:
  • MSN Degree required
  • Graduate of an accredited Nurse Practitioner program required

License/Certification:
  • Current State of California Registered Nurse and Nurse Practitioner and Furnishing License required
  • National certification or eligibility for certification as an NP required
  • Current ACLS certification from the American Heart Association required
  • DEA license required (or eligibility to obtain)
  • AGACNP preferred

Experience:
  • A minimum of 3 years of acute care nursing experience preferred
  • Nurse Practitioner with relevant acute care experience preferred
  • Heart Failure or Cardiac ICU experience strongly preferred
