The HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT has an outstanding opportunity for a PART-TIME MEDICATION SAFETY PHARMACIST (50% FTE, Day Shifts). The Medication Safety Pharmacist (Pharmacist 4) is responsible for management of the pharmacy department’s quality assurance programs.  This position independently provides accountability for the program area, functioning with a high degree of autonomy, and offering expertise in the program area inter- and intra-departmentally and to the community.

PRIMARY RESPONSIBILITIES:
• Manage the patient pharmacy’s quality assurance program.
• Review and evaluate all medication related Patient Safety Net (PSN) reports with the area managers.
• Prepare patient safety and quality reports for the pharmacy department.  Present this data to hospital committees as assigned.
• Prepare quarterly and annual summary reports of PSN data.  Present this data to hospital committees as assigned.
• Use trended PSN data to make recommendations to the pharmacy administrative team regarding system improvements.  Perform follow-up analysis of changes to ensure effectiveness.
• Assist with root cause analysis of high harm score PSN reports related to medications.
• Assist with the annual review of the departmental policy and procedure manual.
• Run reports from the electronic health record databases to meet the needs of the Medication Safety program.

REQUIREMENTS:
Three years experience in pharmacy practice. Months spent in the completion of an American Society of Health-System Pharmacists accredited residency training program may substitute month-for-month for the experience. 

LEGAL REQUIREMENT: State of Washington Pharmacist license.
