Description:
Provides therapy and other behavioral health interventions to program and Department of Pediatrics participants and families as assigned. This includes participants of varying ages and socioeconomic statuses. Assists with, and provides services, for other related business as needed focused around care coordination and service delivery for families with medical, developmental, and behavioral health needs. As needed, supports staff through patient crisis; provides clinical supervision to social work student interns, as needed, and assists with meeting overall programmatic goals around behavioral health.
Duties & Responsibilities:

  • Provides assessment screening and intervention within the scope of training and in accordance with program and departmental policies and procedures.
  • Identifies needed and available resources for meeting social and emotional needs and provides to patients, as needed, with the goal of encouraging in engagement in appropriate services.
  • Documents patient related activities and makes reports as required. This includes timely completion of documentation required for compliance and billing.
  • Responds to patient related crisis, within scope of training, and appropriately engages leadership and other team members to provide support.
  • Participates in plans for cooperation with local health, welfare, and community agencies for more effective services to the total patient group.
  • Provides services through other CHECK initiatives, as needed. Services focus around care coordination and patient engagement for varying ages (children and adults).
  • Participates in and develops orientation courses, staff development programs, and training of program staff and students, as needed.
  • Perform other related duties and participate in special projects as assigned.

  • Minimum Qualifications Required:
  • Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
  • Current State of Illinois Licensure as a Licensed Clinical Social Worker (LCSW).
  • Demonstrated experience with providing behavioral health interventions in a medical or clinic setting.
