Position Description:

The Division of Infectious Diseases is looking to hire five Medical Case Manager I positions. These positions will serve as part of UofL Ryan White Kentucky Care Coordinator Program (KCCP) Medical Case Management Team. The Medical Case Manager I will provide overall case management, support and advocacy for UofL Ryan White eligible clients who adhere to stated program service and enrollment requirements. The goal of the Medical Case Manager I is to aid in removing barriers to care access and to drive improved client outcomes across the KIPDA Region as part of UofL Ryan White Programs.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Maintain an active client caseload of 75+ clients by providing non-medical case management that includes counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
  • Conduct initial acuity assessment of assigned client service needs and develop a comprehensive Service/Care Plan, and coordinate appropriate services to achieve objectives.
  • Complete program eligibility documentation including HIV status/diagnosis, residency, and income every six months.
  • Re-evaluate and make adaptations to the service treatment plan, at least every six months, as necessary during the annual eligibility period.
  • Document case management updates in Careware database including types of services provided, encounters/ communication, and duration and frequency of the encounters.
  • Assist patients to access other public and private programs for which they may be eligible.
  • Coordinate and follow up on medical visits, labs, and assessment of viral load suppression.
  • Provide clients with educational information regarding disease transmission and prevention methods and encourage a healthy lifestyle over the course of continued case management.
  • Participate in ongoing HIV education and attend Ryan White Part B services conference and seminars.
  • Participate in clinical quality management activities as assigned.
  • Other duties as assigned.
Compensation will be commensurate to candidate experience.
