85% Professional Practice Activities

A. Evaluates for appropriateness and approves new drug orders in collaboration with other pharmacists, technicians, nurses, advanced practice providers and physicians

B. Screens for drug interactions (including drug-drug, drug-herb/dietary supplements, drug-food interactions)

C. Prevents, detects, manages, and documents adverse drug reactions to minimize adverse events in patients

D. Maintains accuracy and timeliness in drug dispensing to ensure a high level of patient care and customer service

E. Responds to drug information requests from all internal and external customers

F. Completes pharmacokinetic and therapeutic consults and recommends alternative therapy choices when appropriate

G. Successfully be granted and maintain applicable pharmacy privileges through the OSUWMC medical staff

H. Utilizes pharmacy privileges regularly in providing evidence-based clinical care and elevating practice and successfully completes accompanying competency exams. Available privileging responsibilities include:

a. Adjusts medications based on route of medication administration access

b. Monitors and adjusts medications based on renal, hepatic, antithrombotic indications, and hematologic parameters

c. Orders and adjusts laboratory tests related to monitoring medication therapy

d. Modifies and adjusts Total Parenteral Nutrition therapy and associated fluids and electrolytes in conjunction with a dietician

e. Adjust and modify chemotherapy regimens based on patient-specific parameters

f. Monitors laboratory values and adjusts supportive medication for high-dose methotrexate management

I. Recommends and plans drug therapy considering clinical appropriateness, safety, efficacy, and cost

J. Attends providers’ service rounds if applicable

K. Maintains and reviews patient profiles

L. Participates in Code Blue Emergency Response and Stroke Alert Response as required and subsequent to appropriate training

M. Interacts with physicians, nurses, and other allied health professionals in a courteous and professional manner

N. Demonstrates ability to work as a collaborative member of the team by exhibiting quality interpersonal skills, remaining flexible, and approaching work and collaborators with open-mindedness

O. Maintains productivity of self and fosters efficient operations in a dynamic and changing environment

P. Participates in cross-coverage of adjacent services

Q. Performs patient education in collaboration with nurses, physicians, and other health care professionals

R. Maintains clinical competency through Continuing Education (CE) programs, seminars, published literature, and outside readings

S. Commits to personal growth and development

T. Meets commitments and responds to requests in a timely matter

10% Teaching, Educational, and Supervisory Activities

A. Manages personnel and supply resources to promote efficient, cost-effective operations

B. Contributes to the annual performance review of staff by providing objective and timely performance feedback of peers to managers

C. Maintains organized workspace to help foster efficient operations

D. Performs tasks efficiently and within established time standards

E. Able to deal with a dynamic and changing environment

F. Manages time wisely and demonstrates the ability to prioritize assigned tasks

G. Supports team and management initiatives

H. Conducts experiential teaching of Pharm.D. students, interns, externs, pharmacy residents, and regular staff as assigned

I. Educates medical residents, fellows, physicians, advanced practice providers, and students on correct drug use, prescribing, and policies

J. Possesses global understanding of department operations, workflow, and medication distribution

K. Proactively identifies and resolves operational barriers

L. Participates in recruitment of new hires as assigned

M. Orients and trains new pharmacy team members

N. Assists in development of training materials or checklists if assigned

O. Provides mentorship to new hires and new preceptors

P. Contributes to development and implementation of the strategic plan

Q. Develops and presents clinical pearls to pharmacy staff or nursing units as assigned

R. Provides feedback to technicians and pharmacists working at same site/area

S. Assists with maintaining accurate inventory

5% Research, Project and Quality Improvement Activities

A. Participates on hospital and/or department committees as assigned

B. Identifies and presents ideas for process, medication safety, and quality improvement opportunities

C. Records and monitors Quality and Service Indicators and provides input to managers regarding quality improvement

D. Shows attention to detail, accuracy, and thoroughness in an effort to continually improve quality

E. Participates in department’s Just Culture approach to medication safety

F. Participates in research, medication safety, and quality improvement projects as assigned
