Position Focus:


In 1971, Yale University established Yale Health to provide health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Our state-of-the-art facility at 55 Lock Street, is where our members receive most of their care from our 150+ providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15-bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MRI and CT scan, x-ray and ultrasound and a full-service retail pharmacy.

The Advanced Practice Provider will serve as a Primary Care Clinician in the Internal Medicine Department of Yale Health. Yale Health is a National Committee for Quality Assurance-recognized Patient Centered Medical Home. As part of a Primary Care Team, provide comprehensive whole-person primary medical care to an assigned panel of patients, including preventive care and physical examinations, assessment and management of acute issues, appropriate monitoring of chronic illness and assessment of emotional health and wellbeing; recommend referrals to specialists as needed, consults as needed with physician team members; provides rapid access care, preoperative assessments and supportive care as needed for non-panel patients on the primary care team; participate in cross-coverage for absent primary care team members; commit to medically appropriate care that is patient-centered, evidence-based, compassionate and cost-effective. Abides by departmental and organizational standards of patient service and responsiveness, including but not limited to timely and appropriate communication of test results and timely response to patient messages.

Essential Duties

1. Examines patients to collect information about their physical condition. 2. Obtains, compiles, and records patient medical data, including health history, progress notes and results of physical examination. 3. Conducts complete physical examinations; provides medical evaluation and treatment to patients; develops treatments plans and monitors patient’s progress. 4. Advises patients on results of laboratory tests and recommends referrals to specialists or other medical facilities. 5. Collaborates with physicians, nurses, counselors and other medical staff to provide the best medical care. 6. Provides preventive health practices to patients to maintain and improve health and prevent illness. 7. Coordinates patient care including ordering of laboratory tests and radiographic studies. 8. Prescribes therapy or medication with physician approval. 9. May perform other duties as assigned.


Required Education and Experience

Graduation from a Physician Assistant (PA) Training Program accredited by the American Medical Association Council on Medical Education or Master’s Degree from an accredited Advanced Nurse Practitioner program.


Required Skill/Ability 1:

Well-developed assessment, interviewing, counseling, problem-resolution and crisis intervention skills. Well-developed oral and written communication, organizational and training skills.


Required Skill/Ability 2:

Comprehensive knowledge of state, federal, and university regulations related to administering medications and maintaining appropriate medical records.


Required Skill/Ability 3:

Ability to adhere to organizational standards of medical documentation with the Yale Health electronic health record (Epic) including but not limited to, timely completion of notes, appropriate documentation of non-visit related transactions, appropriate and timely management of Inbasket.


Required Skill/Ability 4:

Excellent attendance, punctuality and reliability.


Preferred Education, Experience and Skills:

Three years of experience in clinical practice, preferably in adult primary care.


Required Licenses or Certifications

CT APRN license (or eligible) with ANA certification OR CT PA license (or eligible). Prescriptive privileges (or eligible) in the State of Connecticut, federal DEA license and CT DCP license for prescription of controlled substances.


Physical Requirements

While performing the duties of this job, the work involves sitting, standing, walking, bending, and occasional slight lifting. The employee occasionally lifts and/or moves up to 50 pounds when transferring a patient from chair or wheelchair to exam table and turning patients for examination. Proper body mechanics must be used when maneuvering patients.
