ORGANIZATIONAL SUMMARY:
The vision of the USF Diabetes Center is to provide patient care, education, and research to youth and adults with diabetes and their families. As a state mandated diabetes education site, the Center provides diabetes education courses to hundreds of people with diabetes and their families each year. Professional continuing education seminars are also offered regularly. Psychology and counseling services are also available within the Center for patients with diabetes. The unit currently includes professionals in the areas of nutrition, diabetes education, psychology, clinical social work, public health, and research coordination. Physicians with a focus on diabetes are also affiliated. In the future, the Center will grow to provide complete, interdisciplinary care for people with diabetes. The center is under the direction of Dr. Jeffrey Krischer, Distinguished Professor of Pediatrics, Pediatrics Epidemiology Center.
POSITION SUMMARY:
A Human Services Practitioner provides direct intervention, action research, training, and consultative services for/to agencies, families, and individuals with various social services programs, developmental disabilities and/or special needs. This position requires a qualified human services professional who will provide direct assistance to patients and families, or serve as case manager to coordinate the delivery of services to patients and families. 
RESPONSIBILITIES:
Plans, organizes, and implements social work services for patients and families in the Diabetes Center. Presents awareness training and activities to the public, including developing and providing materials.
Interviews patients and family members to obtain information on home environment, family relationships, employment history, health history, insurance coverage and patients personality traits.
Evaluates information obtained in terms of the medical plan of treatment and plans a program/action plan that provides maximum benefit including the transition of clinical care from pediatric to adult diabetes care providers.
Assists patients and families to understand, accept and follow medical recommendations. Identifies potential barriers to compliance with medical management.
Provides information and refers patients and families to other community agencies that may assist them. Works collaboratively with school district nursing staff, families and diabetes care providers to provide optimal care to children with diabetes at school. Works closely with school administrative staff in recommending solutions to improve diabetes care at school.
Documents appropriate information in the patient's medical record.  Participates in team review of patient's progress.
Performs other duties as assigned.
POSITION QUALIFICATIONS:
MINIMUM:
Each position assigned to this classification must require specific training at the Bachelor's degree or Master's degree level in Education, Special Education, Psychology, Social Work, or other field appropriate to the program content. Bachelor's degree candidates must have at least two years of applicable experience. The Master's degree may substitute for the required experience. Specific certifications or licensures may be required or preferred for certain program areas.

Four years of direct experience can substitute for a Bachelor's degree or six years of direct experience can substitute for a Master's degree (SB 1310).
PREFERRED:
Extensive knowledge of health care social work.  Extensive knowledge of community resources.
