Job Description Summary:
Project Eagle is a program of the University of Kansas Medical Center’s Institute for Community Engagement focused on preparing children, engaging families, and promoting excellence for children and families in Wyandotte County, Kansas. Project Eagle provides home-based and center-based services to pregnant women and/or children ages birth to five through a variety of programs, including Early Head Start, Connections, and ABC Home Visiting. The Health Coordinator is responsible for coordinating services for health, nutrition, and oral health to ensure the overall health and well-being of Early Head Start enrolled families. The Health Coordinator also works directly with all pregnant women and children with special health needs to provide an additional layer of services and support in the area of health. The Health Coordinator provides training and home visit support to staff and families in the areas of overall child health, breastfeeding, nutrition, and oral health.

Required Qualifications


  • Bachelor's degree in Health Education, Community Health, Nursing, or closely related field.

  • Experience in maternal-child health, pediatric, family health care and/or public health field.

  • Demonstrated experience working with underserved pregnant women, children, and their families from diverse ethnic and cultural backgrounds.

  • Ability to communicate effectively and establish partnerships with co-workers, program families, and community members.

  • Willingness to work in a highly collaborative environment.

  • Desire for continued professional growth.

  • Excellent oral and written language skills. Valid driver's license and access to personal car.  




Preferred Qualifications


  • Master's Degree in Public Health, Health Education, or related field.

  • Bilingual English/Spanish.

  • Experience in the following: Providing Health Education and training for program staff and/or families with children ages 0-5, Primary and preventive health care education for families.  




Key Roles and Responsibilities:


  • Monitor and visit all pregnant women enrolled in the Early Head Start program. Supports home visitors in assisting all enrolled pregnant women in accessing prenatal care and education.

  • Visit all enrolled children and families with special health care needs to provide an additional layer of support in the area of health.

  • Ensure vision and hearing screenings are completed as required by Head Start Performance Standards.

  • Work with staff to identify high-risk infants and/or health situations within families that require more intensive health services and visit those families as needed to address concerns and make appropriate health referrals.  

  • Assist staff members in the development of Health Care Plans for enrolled children. 

  • Ensure staff have appropriate resources and curriculum to deliver health education. Work with staff and families to emphasize the importance that having a medical and dental home, regular well-child exams, immunizations, and dental exams, has on the child’s overall health and development.

  • Identify health, dental, emergency, and nutrition service resources within the community and help parents in securing a source of accessible health care for their families, as needed.

  • Coordinate or facilitate professional development and training to staff in the areas of overall child health, breastfeeding, nutrition, and oral health.

  • Order health supplies and educational materials and manage inventory.

  • Participate in Child and Family Reviews and Team Meetings and work cooperatively with all staff.

  • Participate in the development of program policies, service delivery plans, and ongoing monitoring of program quality.

  • Ensure all assigned program area reports are completed and submitted on a timely basis.

  • Ensures all confidential information is protected.

  • Act as liaison with community health care providers to ensure coordination of services for Project Eagle families. 

  • Participate in the agency Health Services Advisory Committee.

  • Participate in and contribute to management team and all staff meetings.

  • Attend interagency meetings, Parent Committee and Policy Committee meetings, and other meetings, as appropriate.

  • Attend and participate in continuing education, career and professional development opportunities as appropriate to keep abreast of current issues and trends related to the responsibilities of the job.

  • Participate in the annual program self-assessment and ongoing monitoring.

  • Supervise students participating in rotations, internships and practicums at Project Eagle.

Anticipated Salary Range: $50,000 - $55,000
